I NC.NA-AP.ZZ-0015(Q) e SAFETY TAGGING PROGRAM k 1.0 PURPOSE CD-147Y To establish the controls and responsibilities for the Safety Tagging Program which, in turn, provides for the protection of station personnel and equipment.

l 2.0 SCOPE l This program provides for the protection of personnel l

and equipment, identification of equipment in a controlled status and helps to prevent the inadvertent j operation of this equipment.

l This program shall be utilized before beginning " work" on station equipment such as rotating machinery, pressure vessels, electric circuits, electrical equipment, piping, air or gas ducts etc. " Work" is j defined as any activity which could jeopardize the availability of the component / system or present a safety hazard to the worker.

This program establishes the procedure which should be used by the Nuclear Department to ensure the proper isolation of equipment inside and outside of the Protected Areas of the Salem and Hope Creek Generating Stations.

3.O RESPONSIBILITIES l 3.1 Department Heads shall:

o Ensure that all personnel affected by this program attend safety tagging training classes annually j and/or whenever a change in these rules takes

( place.

l o Submit an Approved Tagging List Change Request (Form NC.NA-AP.ZZ-0015(Q)-7) to the Operations Manager for the addition or deletion of personnel to the Approved Tagging List ( ATL) .

o Ensure the ATL is current by processing a Change Request (Form NC.NA-AP.ZZ-0015(Q)-7) and deleting those personnel who no longer require a need for tagging authorization, o Ensure that, prior to an individual in their

  • i department being transferred or leaving the l company, that individual promptly reviews all of their outstanding Tagging Requests.

o Retain all documentation of Safety Tagging Program (STP) training for personnel under their direction.

3.2 Operations Manaaer shall:

o Review and authorizes all Approved Tagging List l Change Requests.

o Ensure the Tagging Request Inquiry System (TRIS) and the standard tagging points list is maintained and updated as necessary.

o Notify the Electric System Operator when '

j personnel are approved to tag equipment greater i than 10KV. l 1

o Maintain an Approved Tagging List for selected 4 personnel who are required to work on equipment l at the station.

l o Schedule monthly and quarterly reviews of active tagouts.

)

i 3.3 Hgg .7ar - Nuclear Trainina shall administer a training i program that provides the following: l o Safety Tagging Program (STP) indoctrination as part  ;

of the General Employee Indoctrination Program. j o Full indoctrination of the Safety Tagging Program for new supervisors that includes:

o The purpose of the Safety Tagging Program.  !

l \

l o A description of the various safety tags.

o Responsibilities of all individuals, with 1 respect to safety tagging. l o Rules applying to safety tagging.

o Yearly requalification training for supervisors on l the Approved Tagging List. This training should be an abbreviated version of the topics comprising training for new supervisors.

o All station operators shall receive Yearly requalification training on the safety tagging system. The training shall focus on the specific i duties for each level of operation but shall also j include a review of the Safety Tagging Program.

NC.NA-AP.ZZ-0015(Q) o Maintain an Approved Non-station Tagging List for selected personnel who are required to work on equipment at their location.

o Approve all Non-Station Tagging Requests and Releases for their jurisdictional area.

3.4 Manaaer - Site Protection or his desianee shall:

o Maintain an Approved Non-Station Tagging List for selected personnel who are required to work on l equipment at their location o Approve all Non-Station Tagging R= quests and Releases for their jurisdictional area.

3.5 The Senior Nuclear Fire Protection Sunervisor (SNFPS) shall:

o Perform all responsibilities assigned to the Non-Station Tagging Authority at Artificial Island.

o Limit the request to include only the tagging necessary to make the equipment safe for work.

l 3.6 The Nuclear Fire Protection Sunervisor (NFPS) shall:

o Review all Non-Station Tagging Requests and Releases to ensure the adequacy of the tagging to protect personnel from' energy sources and equipment from damage.

o Enter the applicable information on the blocking tags in accordance with the Tagging Request.

o Examine the Non-Station tags removed by the Fire Protection Department to verify that all tags have been accounted for/ properly completed.

3.7 , Fire Protection Onerator (FPO) shall; o Isolate, tag and restore Non-Station equipment to service.

o Record time, date and sign the back of the safety tags for placement and removal.

o Perform the necessary operations to safely clear and tag the equipment in accordance with the i request.

l l o Release the tags and reposition all components in t

accordance with the Temporary / Partial Release Request.

( tests to safely return the equipment to service.

3.8 _ Senior Naclear Shift Suoervisor/ Nuclear Shift l Eggervisor (SNSS/NSS) shall:

o Review / approve all tagging request and releases for work on equipment.

o Ensure that when the work and applicable testing of the equipment is complete, the equipment is returned to service.

3.9 Work Control Center (WCC) norsonnel shall:

o Select the type of tagging (Red Blocking tags, l

, etc.) that will be used and enter the information I on the Tagging Point List. This shall include the desired tagged position and a description of each tagging point. The individual points and their description need noe be completed if a standard request can be used. The standard request number shall be entered on the form.

o During power operation, the Work Control will normally identify tagging blocking points for systems or component outages. Additional or special blocking points may be identified by Station Planning. During a forced or planned unit j

outage, outage Planning may be tasked with identifying all blocking points.

3.10 Reauestina Sunervisors shall:

EQZE A. The requesting Supervisor should coordinate with Planning and Operations, as necessary, to avoid redundant and/or unnecessary tagging.

B. The Requesting Supervisor need not be on the ATL.

i However, the Job Supervisor (s) accepting j responsibility for the tags and authorizing

{ release of the tags shall be on the ATL.

{( (Form NC.NA-Ap.ZZ-0015(Q)-1) for notification of tagging completion by the Work Control SNSS/NSS.

1 NOTE i

When requested, the Work Control SNSS/NSS shculd make one attempt to notify the Job Supervisor of the tagout completion. The supervisor in charge shall follow-up on the tagout status in a timely manner. This notification is not required for group tagouts.

o Submit the proposed tagout to the Work Control l Coordinator Group for review and approval.

A. If the Work Control SNSS/NSS is acting as the 1 Requesting Supervisor, the Duty SNSS/NSS I should perform the independent review and approval of the Tagging request.

B. The name (not the position) of the requesting Supervisor and Approving Supervisor are required on the Tagging request to attest to the adequacy of the blocking points.

3.11 Shift Radiation Protection Technician (SRPT) (Hope Creek only) shall:

H2TE The addition of the SRPT to a Group Tagging Request is to ensure the affected room or area is properly posted and/or reposted when the tags are cleared. The technicians are D21 safety tagging for the purpose of equipment or personnel safety; therefore, periodic training is not required.

o Evaluate and performs any posting or reposting of the affected area or room in a;cordance with SA-AP.ZZ-046(Q).

o Upon notification from the Work Control SNSS/NSS, signs off the group tag and coordinates reposting of the area with the Work Control SNSS/NSS.

] 3.12 Job Suoervisor shall:

3.13 Oualified Individual Performina Second Verification shall:

, o Perform component verification in accordance with NC.NA-AP.ZZ-0005(Q), Station Operating Practices j 3.14 All Personnel shall:

l Appropriate disciplinary action will be taken for

those in violat' ion of this procedure.

o Report all tags found unattached immediately to the j Nuclear Shift Supervisor.

4.O PROCESS DESCRIPTION 4.1 Station Tagging Equipment that is to be worked on shall be identified and the appropriate tagging request shall be prepared.

Provisions for Group Tagging of equipment in order to

' reduce redundant tagging and radiation exposure, etc.

The tagging request shall be reviewed and approved by the Operations Department. Prior to the performance of the work activity, the equipment will be prepared, cleared and tagged, independently verified when applicable, by the operations Department.

Job Supervisors shall ensure the equipment has been I appropriately tagged and is safe to work on before * '

beginning the work activity. The work shall be performed and the Job Supervisor shall release their tago. Provisions for temporary / partial tagging releases of equipment for the purpose of testing, etc. is addressed.

The Operations Department shall ensure the appropriate i

operability testing of the equipment is performed and return the equipment to service.

l 4.2 Non-Station Tagging The rules contained in this section are fundamental to the operation of the non-station safety tagging program.

In order that the system will function effectively and fulfill its purpose these rules shall be adhered to by all affected personnel.

o Verify that the number of tags removed is the same as the number of tags requested to be released.

o Return the Partial Release Request with the original Tagging Request to the Tagging Authority.

o Individuals who either request equipment outages or are in charge of the work being performed shall be authorized by a non-station Tagging Authority and their name(s) placed on the Approved Tagging List.

o The person requesting an equipment outage shall indicate all blocking points on the tagging request.

o Equipment tagged with a Worker's Blocking Tag SHALL NOT BE OPERATED by anyone except the individual named on this tag.

o The Supervisor must be notified prior to releasing tags from a piece of equipment. The Supervisor must then assure that all personnel working on the i equipment have been notified of the tag release and are safety clear of the equipment.

o Tags SHALL NOT BE REMOVED UNTIL APPROVAL (in writing) is granted by the tagging Authority under whose jurisdiction the work is performed, or their designee.

o Equipment shall NOT BE RESTORED TO SERVICE until all tags have been removed.

5.O PROCEDURE 5.1 General Taccina Rulqg 5.1.1 A fundamental rule of the tagging program is "the person named on the tag is the only person authorized to request the removal of the tag." If the person has left the site, the release may be authorized by Nuclear Common Page 9 of 54 Rev. 0

4 affected unit and the person identified on the request.

In the event the person cannot be located, only the General Manager-Salem or Hope Creek Operations, the Operations Manager or the Operating Engineers shall have the authority to sign (or authorize the Senior Nuclear  !

Shift Supervisor / Nuclear Shift Supervisor to sign) the l Tagging Release. This shall be done only after receiving the strictest assurance from the .

cognizant department supervision that all persons who were working under the protection of the tag (s) are safely clear of the equipment and the equipment is in condition for service.

5.1.2 Anyone requesting a list of components that have i been tagged for a particular job, shall have the list  ;

provided by the Work Control Center. l l

5.1.3 Group tagging should be used, when possible, to reduce l duplicate tagging of equipment and therefore reduce I radiation exposure, as applicable. Group tagging shall only be used for the application of Red Blocking Tags.

i 5.1.4 Red Blocking Tags, White Caution Tags, Yellow l Permissive Tags and Worker's Blocking Tags shall only be used when associated with an authorized Tagging Request.

They shall be applied and removed by authorized Operations Department personnel.

5.1.5 Entire mechanical systems or parts.of systems where energy sources are present may be tagged as unit systems with red blocking tags placed at the isolating boundaries. Any drains, vents, relief devices or other devices which could, under certain conditions, allow energy in any form to accumulate CD-447A shall be tagged in the SBSD_DStEllistD with a Red '

Blocking Tag. If the vent ox drain is the component to be worked or will be removed from the system, a red blocking tag should not be applied if within the boundary isolation there exist other vents or drains which could be used to ensure the system is depressurized. However, if no other vent or drain is available, the tags should be hung on the vents or drains to be worked and the job supervisor shall either partially or temporarily release those tags which interfere with the work, only after the supervisor has determined that the system is safe and depressurized.

1 1 shall have tags temporarily released, except for the repacking of a valve on the backseat. If the valve to be worked is a boundary valve, the boundary has to be expanded j to include this valve.

5.1.7 Red Blocking Tage on valves which are to be removed shall j be partially or temporarily released.

j 5.1.8 Job supervisors who have personnel working on tagged i equipment under their supervision shall have that equipment j tagged in their name or in accordance with a Group Tagging i Request.

l 5.1.9 If more than one (1) job supervisor performs the task of

{ tagging verification prior to work performance, each job

supervisor shall initial the tagging points that he/she i verifies on the Tagging Point List, Form 4 NC.NA-AP.ZZ-0015(Q)-5 or on a copy of the TRIS taggout

! worksheet.

) 5.1.10 After the clearing and tagging of equipment, either i initially or following a temporary. release, the supervisor

. in charge of the job shall visually check the placement of l all necessary tags and the proper positioning of all

equipment, prior to commencing work. This shall be done
to ensure that all energy isolation devices are properly 5 cleared and tagged. When an isolation point is remote from the work location or is inaccessible due to other reasons,

. the authorized person shall ensure the proper positioning of equipment and placement of necessary tags through verbal

! communication with the tagging authority. For Group I '

Tagging, the Job supervisor is to verify proper placement of necessary tags and positioning of all equipment required i for their work and ensure the safety of their workers.

4 4 5.1.11 When tagging is done only to protect the equipment and no i

work is being done on it, an Operations Department j . supervisor's title, such as Senior Nuclear shift supervisor j or Nuclear Shift Supervisor, should be used.

4 5.1.12 In no case shall the requesting supervisor and the

j. approving Nuclear shift Supervisor be the same person.

{ 5.1.13 Equipment tagged only with a Red Blocking Tag shall not be

! operated by anyone until the tag has been properly released and removed.

]( operated by anyone until all special instructions are complied with. {

i 5.1.15 Every Yellow Pereissive Tag shall have a red blocking tag l l

1 between it and any source by which the equipment could be i energized. This applies, except when used in accordance i

with'Section 5.17.13 for Motor Operated Disconnects / Circuit j Switches. l 1

j 5.1.16 No tags shall be removed until the equipment has been l

i CD-648A released for service by the person for whom it has been '

l tagged. IN NO CASE SHALL ANY EQUIPMENT BE RESTORED TO l

SERVICE UNTIL ALL RED BLOCKING TAGS AND YELLOW PERMIS8(VE

I 5.1.17 Work shall not be performed on high voltage electrical i equipment that can be operated for test or adjustment without the apparatus being tagged with a Yellow Permissive Tag.

i 5.1.18 Permission shall be secured from all persons named on the Yellow Permissive Tags before any operation or test of the

! equipment is permitted if there are two or more Yellow Permissive Tags for different individuals on the same equipment.

5.1.19 When a group taggout is generated, the request should be in the title of the responsible department Sector Supervisor, j if known or SN88/N88 by title. The 8N88/N88 or Senior
j. Supervisor shall sign on column 1 of the Group Tagging a

Request until the Job Supervisor takes responsibility for l the tagout. At that point, the 8N88/N88 or Senior i

' Supervisor may sign off the request. No work may be performed until the Job supervisor signs on the request. l 4 5.1.20 When tagging is done only for planning purpoaes (e.g., for l a refueling outage) and no work is being done on the item, a tagging Request shall be initiated by entering l "Prestaged" (P) in the Punction Mode line and Planner in

the " Tagged For" line. When ready to process that Tagging j

Request, the Punction Mode shall be changed to " Initiate" (I) and " Planner" should be changed to the person (s) the 4

request will be tagged for.

If any additions or deletions are required to a tagging 5.1.21 i request, the entire Tagging Request should be returned to l the WCC. The Requesting Supervisor and the Work Control

SN88/N88 should agree to the change. Their concurrence i shall be noted by initiating the border of Form 1

NC.NA-AP.ZZ-0015(Q)-1. At this time, a new TRIS worksheet will be generated and any additional tags will be printed.

for tagging, then tha sequence will be specified on the worksheet, or OD-8 or Attachment 2 of OP-AP.ZZ-109(Q)

5.1.24 Under no circumstances should a permanent tagging release i be made if the Requesting supervisor requests operations to remove the tags but nor reposition the cayaponents. A j temporary release should be utilized for this condition and a the permanent release special instructions on the Form i NC.NA-AP.ZZ-0015(Q)-1, modified to indicate that component

! position has not been changed nor verified.

1 j 5.1.25 Temporary releases and temporary tag release cancellations

! involving rack-in type breakers should be handled in accordance with the note in Section 5.5.

e. Ensure the equipment is released and an independent verification is performed of the release of the equipment in the systems identified in NC.NA-AP.ZZ-0005(Q), Station Operating Practices.

Check the "YES" blank on the Tagging Request, if required. Independent verification is not required for a tag for tag, one for one tag swap.

5.4.4 Nuclear Control Operator shall:

a. Inform the Load Dispatcher if tagging involved work on equipment under his jurisdiction.
b. Instruct the Nuclear Equipment Operator to remove the tags and restore the apparatus to operable status.

c. Confirm the Tagging Release in TRIS
d. Initial the Tagging Release to indicate that the Second Verification is complete, if required and that Tagging Release has been confirmed in TRIS.
a. Notify the Nuclear Shift Supervisor that the Tagging Release'has been completed.
f. Bundle the Tagging Request, associated worksheets, and tags used in a neat package.

5.4.8 Nuclear Shift Supervisor shall:

5.5.1 RACK-IN TYPE BREAKER " TEMPORARY" RELEASES  !

When a job supervisor wishes to either " temporarily" release a breaker from "DI" DISCONNECT or "TE" TEST position, the following format must be utilized.

l 5.5.2 Job Supervisor shall: -

is indicated and submit it to the Work Control Center. The WCC shall assist as necessary.

d. Authorize the re-application of the tags or authorize a permanent release after the testing is complete.

f. Request the Work Control Center to contact the Job Supervisor when the temporary release is complete.

5.5.5 Nuclear Control Operator shall:

The caution tag shall not be used on blocking points for protection of personnel or equipment.

5.10 Worker's Blockina Taas Worker's Blocking Tags provide a means to tag and

" operate" a piece of equipment. The individual named on the tag or persons directed by the individual may operate the tagged equipment as required.

Worker's Blocking Tags are used for: MOVATS, Screen Inspections, Leak Rates (Salem only), Stroking, Main Steam Isolation valves and Relay Work by the Relay Department. WBTs may be used in other applications on a case by case basis when approved by the SNSS/NSS.

5.10.1 The administrative controls of Sections 5.2 through 5.7 of this procedure shall apply to the processing and use of the WBTs except as noted below:

a. The Worker's Blocking Tag shall not be used to isolate equipment from high voltage energy sources.

b. The Worker's Blocking Tag shall not be installed on any blocking point that is already tagged with any safety tag, including Worker's Blocking Tags.
c. Tag swaps are not permitted for WBTs.

5.10.2 The individual named on the Worker's Blocking Tag or l

the person directed by the individual may operate the tagged equipment as required following the tagging operation. The " tag" shall remain affixed I

and the equipment RETURNED TO THE TAGGED POSITION until a Tag Release is authorized.

5.11 . RLRCTRIC SYSTEM OPERATOR (ESO) TAGGING Group tagging may he performed with the ESO's permission. A supervisor may be added to the ESO group request by contacting the ESO.

5.11.1 Initiating the Request

a. The initiator shell complete a Request for and Record of Equipsant Outage form (Attachment 1) and submit it to the Work Control Center.

voltage switches applied in accordance with a Tagging Request should be tagged with a Red Blocking Tag.

l 5.17.7 Draw-out type circuit breakers should have red blocking tags hung on the racking device when practical, or on the cubicle door for all other cases. Yellow Permissive Tags should be hung on the manual operating handle of the breaker.

E1221 Draw-out type breakers should only be tagged in the

" TEST" or " DISCONNECT" position. Tagging this type of breaker in the "OPEN" position is not an acceptable designation.

) d. When a 480 VAC, 250 VDC, or 125 VDC AC is to be removed from the cubicle, handling of Red Blocking

Tags shall be performed in a similar fashion. If a
tag exists on the racking screw, a PSE&G i Electrician may remove the breaker from the cubicle 4

and transfer the tag to the tape / rope or enclosure l cover. Reinstallation of the breaker into the cubicle shall be performed similar to a 4 KV or 7.2 l KV breaker.

, e. When a draw-out type circuit. breaker is withdrawn I from its connected position, and the only work l being performed is on the breaknr itself, a red 5

blocking tag for the Job Supervisor will be placed

} in accordance with paragraph c. or d., above.

. l l 5.17.8 When electrical apparatus which cannot be physically l i racked out is being used as an isolation point, both the '

i device and its associated contro) circuits shall be

! cleared and tagged with a red blocking tag.

J j' 5.17.9 When tagged, open cartridge fuses or tneraal overloads shall be removed and placed in a plastic pouch with a safety tag. The plastic pouch should have no metal components to eliminate possible short-circuit hazards.

Fuse blanks shall be placed in fuse holders and the plastic pouch containing the safety tag and fuses should be attached to the fuse blank.

5.17.10 If a cap or lockwire is removed, the Nuclear Equipment operator should place the removed item (s) in a plastic bag, pass the tag string through the bag hole, and attach the beg to the nearest appropriate blocking point. This ensures that the cap and/or the required length of lockwire will be available upon restoration.

f l 5.17.13 Motor Operated Disconnects / Circuit Switcher - The device shall be properly isolated from energy sources

' on the high voltage side of the apparatus using Red Blocking Tags. Additionally, three Yellow Permissive Tags shall be placed as follows:

The use of the Yellow Permissive Tag on the low voltage i control energy isolation device does not require a Red j Blocking Tag between it and the low voltage source by 3

which the control circuit could be energized. This will allow the person named on the tag to operate the device as necessary.

5.17.14 Air Operated Valve Tagging CD-257E Air operated valves shall not be used as mechanical blocking points for safety tagging unless no other practical method of accomplishing the required tagging exists.

a. Tagging an air operated valve that fails closed upon loss of air.
1. Close the air operated valve.

3. If the valve has a jacking device, apply the jack to the closed position.
4. Apply a Red Blocking Tag to either the instrument line connection at the valve operator or to the handwheel, if one exists.
5. The appropriate special instructions shall be on the tagging request.
b. Tagging an air operated valve that fails oper upon loss of air.
1. Close the air operated valve.
2. Apply a Red Blocking Tag to the closed air operated valve instrument line connection at the valve operator.

3. Apply a Red Blocking Tag to the OPEN air supply valve.
4. With the air operated valve in the closed position, install a mechanical restraint on the valve to maintain the valve in the closed position.
5. Apply a Red Blocking Tag to the uschanical restraint.

c. Tagging OPEN an air operated valve that fails closed upon loss of air.
1. Open the air operated valve.
2. Apply a Red Blocking Tag to the closed air operated valve instrument line connection at the valve operator.

4. With the air operated valve in the OPEN position, install a mechanical restraint on the valve to maintain the valve in the OPEN position.

5.17.15 Tagging of a Freeze Seal

a. Freeze seals used as blocking points shall be i included on the Tagging Request as a blocking point,
b. A Red Blocking Tag shall be attached to~the freeze seal encapsulation.
c. The supervisor in charge of ths freeze seal, (whether PSE&G or Contractor), is responsible for maintaining the freeze seal integrity while the freeze seal is being used as a blocking point.

5.17.16 Tagging of a Blank Flange

a. A Red Blocking Tag shall be applied to the blank flange, or " pancake" flange used to provide for blocking / isolation of lines to facilitate work performed downstream of the installed blank flange.

6.11.3 Yellow Permissive Tags shall be used only on High Voltage electrical equipment in conjunction with Red '

Blocking Tags. The one exception is when Yellow Permissive Tags are used on the low voltage control l source devices such as circuit deions, fuses, test l switches on disconnects or manual operators on 4KV circuit breakors which are cleared and tagged for work, to allow the individuals named on the tag to operate the equipment as necessary.

6.11.4 Every Yellow Permissive Tag shall have a Red Blocking j Tag between it and any High voltage source by which the l equipment could become energized.

6.12 Worker's Blockina Tac (WBT) - a red tag with a yellow stripe that is used for the following purposes:

6.12.1 Identifies the mechanical or low voltage electrical blocking (600 volts or less) points between any circuit or equipment that is energized and the de-energized equipment upon which work is to be performed.

6.12.2 Blocks and prohibits the operation of equipment by an individual other than the worker named on the tag or l persons directed by +.he worker.

6.12.3 Worker's Blocking Tags shall not be used at any clearance point in which other safety tags have been used for any tagging, including other WBTs.
