EC.IC-GP.ZZ-0008(Q) 1.0 PURPOSE 1.1 The purpose of this procedure is to provide Maintenance

( personnel with administrative troubleshooting widance that interfaces with station procedures and policies.

1.2 To provide administrative controls to ensure compliance with NC.NA- AP.ZZ- 0013 (Q) by providing Attachment 2 to document temporary modificat_ ions made IAN other procedures.

1.3 This procedure is not required provided that the  !

troubleshooting is limited to the measurement of '

parameters NOT requiring jumpers or lif ted leads.

1.4 To provide a source of current information via component attachment sheets on known component problems.  ;

2.0 PREREQUISITES 2.1 Administrative 2.1.1 The Job Supervisor has verified that the-individual assigned to perform this procedure is qualified to do so IAN HC.MD- AP.ZZ-0014 (Q) , e -

Maintenance Qualification and Training Program.

2.1.2 The Senior Nuclear Shift Supervisor / Nuclear Shif t Supervisor (SNSS/NSS) has granted administrative authority for the performance of this procedure.

2.1.3 If applirsble, a Radiation Work Permit (RWP) has been obtained.

2.1.4 The work package has been reviewed by the Job l

Supervisor for completeness and any special administrative prerequisites have been noted and addressed.

2.1.5 Applicable personnel and equipment safety precautions have been complied with IAN station administrative procedures.

2.1.6 Working copies of documents used shall be validated and st g ed by the Job Supervisor IAW NC.NA-AP.ZZ-0003(Q).

2.2 System CD-588A 2.2.1 If applicable, ensure the NCO resets any existing half scram prior to the start of any l troubleshooting.

h 2.2.2 Consideration shall be given to Technical

Specification repirements such as minimum i channels required to be OPERABLE, or NOT TRIPPED.

. 2.2.3 Circuits shall be analyzed for inadvertent

} e pipment operation and the initiation of trip i 2

signals caused by the placement of jumpers, or

( lifting leads prior to their use.

2.3 Definitions Test -

A controlled set of plant operations intended to verify that the systems, structures or components function in accordance with predetermined specifications. Such tests would be of significance to the degree that they should be described in the SAR, could affect safe operation of the plant, could degrade the margins of safety during normal operations or anticipated transients, or degrade the adequacy of structures, systems or components used to prevent accidents or mitigate accident conditions. This may include operation of systems or components outside their normal configuration or addition of new components or l systems not described in the SAR.

Experiment - A controlled set of plant operations intended to establish characteristics not previously known. Such experiments would be of '

significance to the degree that they should be described in the SAR, could affect safe operation of the plant, could degrade the margins of safety during normal operations or

, anticipated transients, or. degrade the adequacy of structures, systems or components used to prevent accidents or mitigate accident conditions. This may include operation of systems or components outside their normal configuration or addition of new components or systems not described in the SAR.

3.0 PRECAUTIONS AMD LIMITATIONS 3.1 Ensure that the Nuclear Control @erator (NCO) is instructed as to which specific alarms and indications will be affected by the performance of this procedure.

3.2 Ensure that M&TE used is set to the proper function and range prior to its use.

3.3 IE it is suspected that the test epipment may not be performing properly, THEN a comparison check shall be performed with a similar device QR the device shall be checked for calibration against a standard. This shall be done prior to taking any other corrective action.

3.4 Ensure that all Measuring and Test Equipment (M&TE) has a current calibration sticker, and is controlled IAh NC.NA- AP. ZZ- 0022 (Q) .

3.5 If appropriate, ensure that a manufacturer recommended warmup period is provided for all instrumentation and M&TE.

t EC. IC-GP.ZZ- 000 8 (Q) 3.6 If venting and/or draining an instrument, or instrument line, contain and dispose of all liquids IAW the RWP or established safety practices. .

3.7 Discrepancies noted during the performance of this procedure shall be documented IAW NC.NA-AP.ZZ-0020(Q).

3.8 Devices installed using torque paint on mounting bolts, etc. shall not be removed for bench calibration. In addition, the ASME pressure boundary shall not be compromised. The ASME pressure boundary is identified on drawing 10855-J-G-1010-3.

3.9 Second party (independent - an individual different from the person making the change) verification shall be required for each Q, F or R listed lifted lead, pipe l disconnected, jumper (wire, staple, etc.), or

! instrument valve repositioned under this procedure.

i l 3.9.1 All leads shall be marked (ie - color coded, I conductor number and/or permanent label. IE the leads are not marked in this manner, IEEN a temporary marker shall be installed. The temporary marker shall be removed when the lead is re-connected, a -

l CD-596F 3.9.2 All Relay Department personnel shall fill out Attachment (s) 5 for each listed lifted lead or jumper (wire, staple,'etc..) not listed in HC.RL-PM.ZZ-00011Q).

CD-326G 3.9.3 The number of wires on a terminal post shall be determined, documented, and independently verified prior to altering the wiring configuration on that terminal. The number of wires shall be verified to be correct after restoration of wiring on an affected terminal post. The column provided on Attachment 2 (5 for relay department) shall be used for this purpose.

3.10 When a difference between the installed epipment and CD-136A the documents being used for troubleshooting exists,

, resolution shall be obtained from the Job Supervisor before proceeding with the troubleshooting effort.

3.11 Only the materials and solvents approved by NC. NA- AP. ZZ- 0038 (Q) may be used in the performance of this procedure.

3.12 When appropriate, equipment shall be properly tagged IAW NC.NA-AP.ZZ-0015(Q) prior to perfo M ng any work.

3.13 Jumpers or lifted leads which must remain at the completion of the work QR if the work activity is stopped for more than two shifts shall be administratively controlled IAW NC.NA-AP.ZZ-0013(Q).

HC.IC-GP.ZZ-0008(Q) 3.14 The use of grounded M&TE shall be limited to CD-691A applications where isolation from power supply grounds are possible unless determined that the M&TEs use will

( not produce adverse effects on the circuits under test.

3.15 Whenever possible, steps shall be taken to troubleshoot in-plant equipment in the de-energized state.

3.16 Lifted leads must be suitably insulated from other circuits and electrical grounds. Exposed electrical leads must be taped or insulated when left unattended.

l 3.17 Electrical jumper cables must be of sufficient length and contrasting color to stand out from other cables.

l The use of " alligator clips", clamps or other l attachments subject to inadvertent disconnecting is discouraged.

3.18 Mechanical jumpers must be marked uniquely to stand out from other hoses or pipes. The temporar

! tag may be sufficient for this purpose.y However, modification in

some cases, additional marking may be needed (additional tags at regular intervals for long jumpers).

3.19 Cogonent attachment sheets must be reviewed for '

applicability in the course of troubleshooting.

4.0 EQUIPMENT REQUIRED The test equipment used in the performance of this procedure shall be as needed. The M&TE used shall be documented on 1

1 the associated work order.

Attachment 1 is optional for de-energized troubleshooting.

5.1 De-eneraized Troubleshootina-CD-302G ,

Troubleshooting shall be considered to be de-energized troubleshooting only when it is performed under conditions where the equipment, channel, component, I etc. is isolated to the point where its active functions (trip / isolation signals, equipment operation, etc.) are inhibited from affecting plant ep ipment or

. operations. Troubleshooting shall be considered l energized when it affects electrical, electronic, t

pneumatic or hydraulic parameters on equipment, systems or components which are not isolated from service. i Normal routine isolation of epipment where the process I is not expected to be affected (eg. operation of instrument isolation valves on non-sensitive racks) is exempted from this definition. Isolation will normally be accomplished via test / bypass switches, electrical ,

breaker operation, fuse removal and/or isolation IAW HC.SA-AP.ZZ-0015(Q). -

5.1.1 Ensure all prerequisites have been satisfied.

5.1.2 Copies of all c e leted Attachments shall be affixed to the validated work document.

(Planned Work Order, Work Request) 5.1.3 When using M&TE, insure the Work Order number is recorded in the MAINTENANCE TROUBLESHOOTING RECORD.

5.1.4 Notify the NCO that the troubleshooting activity is starting.

5.1.5 Troubleshoot, under the following precepts, the equipment to identify the cause of the malfunction.

A. Attachment 2 shall be used to identify all wiring, piping and valve position changes made during troubleshooting. ,

CD-675X B. Perform any independent verification I required. l C. Use sound, logical troubleshooting techniques. l D. The work order, associated procedures and validated documents shall be used at the work location.

5.1.6 The Job Supervisor shall determine whether the troubleshooting effort is to be continued to l

problem resolution or component replacement.

l 5.1.7 Notify.the NCO as to the equipments status

! (i.e., operable or i~noperable).

5.1.8 IE the equipment is HQI to be reworked at this

, time, IHEN address all testing modifications as I

CD-427F D. If it is determined that modifications will CD-492F will be transferred to the administrative control of NC.NA- AP.ZZ-0013 (Q) , the SNSS/NSS shall complete Attachment 3 and notify appropriate Maintenance Planning and System Engineering personnel.

E. Perform any independent verification l required (i.e. another technician,  ;

supervisor, etc. ) .

. F. Clean the area, close all cabinet drawers, I panel doors, etc.

G. Complete all appropriate attachments to this procedure. Attachment 1 is required.

H. Notify the NCO that the equipment is not ,

. operdle and Technical Specification consequences may be applicable.

l I. Submit all work documents - Work Order,

! attachments to this procedure, etc. - to the

Job Supervisor for processing.

l HC o IC-GP . ZZ- 0008 (Q) l 5.1.8 J. The Job Supervisor shall review the Work i documents for completeness, signing where appropriate, then process IAW

( HC. SA- AP. ZZ- 0009 (Q) .

5.1.9 Rework the equipment considering the following guidelines:

A. Use only approved replacement parts.

Substitutions may only be used with Site Engineering approval.

B. Use sound material replacement techniques.

C. Parts tickets, tags, etc. shall be retained and attached to the work order.

D. Notify the Job Supervisor and the NCO when the rework is completed.

5.1.10 Perform any independent verification required.

5.1.11 Perform the necessary recalibration, retest activities to verify proper equipment operation after rework.

e 5.1.12 Clean the work area, close all cabinet doors, panel drawers, etc. IE work war performed on high voltage switch gear, THEh ensure that a thorough close out inspection is performed.

5.1.13 Notify the NCO that the equipment is released from testing or Temporary Modifications are installed.

5.1.14 Ceinplete Attachment 1 if applicable, and submit all other work documents to the Job Supervisor for processing.

5.2 Enercized Troubleshootino l Troubleshooting shall be considered to be energized troubleshooting when it does NOT meet the requirements defined in step 5.1.

5.2.1 Perform step 5.1.1 through 5.1.4.

NOTE 5.2.2 The SNSS/NSS shall act as the Job Supervisor for the Shift Electrician or the Shift Technician when a Maintenance Supervisor is not available.

5.2.2 Using Attachment 1, the Job Supervisor shall prepare a detailed outline of the intended troubleshooting process. The outline shall include such items as what points will be jumpered, leads lifted, staple jumpers moved, valves or switches repositioned, etc. and the expected results of each test.

5.2.3 After preparing the plan, determine if the plan l could cause an ESF system actuation or there is a reactor trip possibility and sign in the spces provided on Attachment 1, and review it (in detail) with the on-shift STA.

5.2.4 The STA will determine if the plan constitutes' the performance of a test or experiment.

5.2.5 IE the STA has determined that the plan is a test or experiment, 22iEN the plan should be.

rejected or modified such that it is not a test or experiment; OTHERWISE the plan will fall under the jurisdiction of NC.NA-AP.ZZ-0008(Q)

' and a Test and Experiment DCP shall be prepared.

5.2.6 IE the STA has determined that the plan is not a test or experiment, THEN after the STA reviews the plan and signs Attachment 1, the STA shall obtain the SNSS/NSS acknowledgment of the Job Supervisors determination of an ESF system actuation or reactor trip possibility and approval for the work to proceed.

NOTE 5.2.7 The NCO shall be notified before performing any steps that could cause an ESF system actuation or reactor trip. l 5.2.7 After approval, the Job Supervisor will review the plan with the Technicians performing the work, pointing out any steps tht could cause an ESF system actuation or reactor trip and l sign on Attachment 1, step 2a.

h BC.IC-GP.ZZ-0008(Q) 5.2.8 The Job Supervisor may proceed with the troubleshooting IAW the approved plan.

Deviations, except AC or DC voltage fi measurements, are not allowed without first providing a modification to the plan IAW steps 5.2.1 through 5.2.7.

5.2.9 From this point onward, steps 5.1.4 through 5.1.16 shall apply. .

ATTACEMENT 6 Agastat Relay Known Problems

3. The acceptable criteria for locking spring engagement using either the old or new style locking clip is as follows:

A. Locking clip is fully engaged at relay base.

B. Locking clip shall have two contact points on the flat, top part of the relay away from the curved edge., "

C. No gap between the relay base and socket. t i
