AFFIDAVIT OF IRVING F. DEWALD 1

1 I Irving F. Dewald, being first duly sworn, hereby depose and state as follows:

1. I an employed by Comstock Engineering. Inc. as Quality control Manager at the Braidwood station.

2. My business address is Braidwood Nuclear Power Station, Braceville, Illinois 60407.

i 3. I have participated in the preparation of the responses to specific Interrogatories Nos. 14, 15, 16, 19, 21 and 53 and the supplemental response to specific Interrogatory No.13, filed by Intervonors Roren 31 31 These responses were submitted in Applicant's Fourth and Fifth Partial Responses to Intervonor's First Set of Quality Assurance Interrogatories and Request to Produce. In particular, I l

have responsibility for (1) the Supplemental response to l Interrogatory No. 13; (2) the portion of the responses to Interrogatories Nos. 14 and 16 that deal with Comstock; (3) the second, third, fourth and fifth paragraphs of the response to Interrogatory No. 15; (4) compiling the information contained in Attachment 2 of the response to Interrogatory No. 19; (5) all but the first and last paragraph of the response to Interrogatory No.

21; and (6) the fourth, fifth and sixth paragraphs of the response to Interrogatory No. 53.

4. To the best of my knowledge and belief, the statements contained in the portion of the Supplemental response to Interrogatory No. 13 as corrected below, the responses to specific Interrogatories Nos. 14 as corrected below, 15 as corrected below, 16 as corrected below.

21 and 53, for which I have responsibility are true and correct.

In addition, the statements contained in Attachment 2 of the response to Interrogatory No. 19 were true and correct when submitted, to the best of my K.awledge and belief.

5. a. In the fourth paragraph of the supplement al response to Interrogatory No. 13. delete the first rentence and the first word of the second sentence and insert the following:

, in the following Procedures:"

b. In the second sentence of the seventh paragraph of the Supplemental response, (1) change the word " Subparagraph" to

" Subparagraphs"; (2) add the words "through 3.3.2.4" after the word "3.3.2"; and (3) change the word " indicates" to " indicate",

c. In the tenth paragraph of the Supplemental response, (1) insert the word " Performance" as the first word of the paragraph; (2) insert the word " Inspection" afher the words " Quality Control" in the second sentence; and (3) add the following as the last sentence in the paragraph:

"Recertification of Quality Ccntrol Inspectors is conducted for each area certified, during a period not to exceed three (3) years, by administering a new General Inspection Proficiency Test per the requirements of section 3.5.1.1.

6. a. In the sixth paragraph of the response to Interrogatory No. 14, change the word " supervisors" to " supervisor".

b. In the neventh paragraph of the response to Interrogatory No.

14, change the words " Quality Control Supervisors" to " Quality Control Supervisor of Inspectors".

c. In the eight paragraph of the response, add the following as the last sentence:

"This individual is trained and tested as defined in Procedure 4.1.2 " Position Delineation" Section 3.29, Subparagraph B."

7. In the. third paragraph of the response to Interrogatory No. 15, delete from the fourth sentence the words " addition of two areas of certification" and insert the words " deletion of one area of certification as not required and addition of one area of certification".

8. In the portion of the response to Interrogatory No. 16 that deals with Comstock, add the words "of Inspectors" afher the words i " Quality Control Supervisor" in the second sentence.
