Dear Mr. Davis:

This report is submitted in accordance with Byron Station Unit 2 Operating License NPT-66 Appendix A, Technical Specification 4.8.1.1.3, reporting of Diesel Generator rallures.

The attached report addresses one invalid Iallure experienced on the 2A Diesel Generator, and one invalid failure experienced on the 2D Diesel Generator.

The criteria to determine valid and nonvalid testa and fallures is in accordance with section C.2.e of Regulatory Guide 1.108.

Only one Diesel Generator was required for operation at the time of the failures. The redundant Diesel Generator was operable during each event.

Since the Diesel Generators were not in service at the time of the failures, they were not considered to be unavailable at. any time as a result of these failures. The failures will have no impact on the testing frequency of the Unit 2 Diesel Generators. The failures are not included in the criteria used to determino the testing frequency as lated in Regulatory Guide 1.108.

9 DIESEL GENERATOR 2A INVALID FAILURE APRIL 7, 1992 On April 7, 1992, during a maintenance run of the 2A Diesel Generater, the fuel injection pump in the 7 right cylinder seized. This prevented the injection of the fuel into the associated cylinder. The engine was

subsequently shutdown and the pump was replaced.

Previous analysis of failures of this type has identified particulate contamination as the cause of pump f ailure. This mode of failure has also occurred at several other utilities and has made this issue a high priority with the Cooper Bessemer Owners' Group. Commonwealth Edison, the pump manufacturer (llanes Corp.), and the supplier (Cooper-Bessemer) are assisting in the investigation.

DIESEL GENERATOR 2D ItWALID FAILURE MARCH 20, 1992

On March 20, 1992 the 2B Diesel Generator was started for a maintenance run i following the 5-year engine inspection. Approximately lo minutes into the run, the operator noticed an abnormal noise coming from the 4 right cylinder.

Further inspection revealed the crosshead roller for the intake valves had seized. The crosshead is part of the rocker arm assembly which in turn opens and closes the associated intake and exhaust valves.

< The failed crosshead was. removed and replaced with a new one. In addition the enm shaf t covers on all 20 cylinders were removed. All crosshead assemblies wero inspected and no other problems were identifled. This is considered an

~ isolated event. No further corrective actions-are pinnned at this time.
