On September 4, 1991, during the execution of the Selsmic Instrumentation 184 Dny Operability Verificat ton Surveillance, tape deck #1 (free fleid sennor) failed to record / play and the system was declared '.noperable.
Technlcal Specification Limiting condition for Operation Action Requirement OliUS 3.3.3-1s was entered.
Tape deck 01 was repaired.
On September 9, 1991 8
topo dock 92 (conta.ument foundation elevation 377' nonsor) failed to second/ play.
Tape deck 12 was subsequently repaired.
On September 16, 1991 sensor 5 (aux 111ary building elevation 426') talled and the playback tape dock faljed to playback. The playback tape deck and sensor 5 were subsequently repaired. On October 9, 1991 the operability surveillance was successfully P" formed and the system was rest ored to operable status.
