Dear Sir,

This report is submitted in accordance with Byron Station Unit 1 Operating License NPF-37 Appendix A, Technical Specification 4.8.1.1.3, reporting of Diesel Generator Failures.

This report addresses one valid test and failure experienced on the 1A Diesel Generator. The criteria to determine valid tests and failures is in accordance with section C.2.e of Regulatory Guide 1.108. Attachment A provides a summary, as prescribed by Regulatory Guide 1.108, of all tests that occurred within the time period over which the last 100 valid tests were performed.

On September 25,1987 at 1327, the 1A diesel generator (DG) was started per

)

the monthly operability surveillance, 1BOS 8.1.1.2.a-1. Approximately thirty i minutes into the full load run the engine operator noticed a seventy dagree fahrenheit temperature drop in the number five right (5R) cylinder. Further inspection of the SR cylinder revealed a substantial fuel oil leak at the '

connection of the fuel pump and the fuel pump diccharge line. The 1A DG was shutdown at 1426 on September 25, 1987. At 1435. Limiting Condition for Operation Action Requirement 1BOS 8.1.1-la was entered for one diesel l generator inoperable. At 2016, repair activities to replace the fuel pump and .

l the fuel discharge line were completed and the 1A DG was started for a

!. maintenance run. During the maintenance run peak firing pressures were

) obtained from the SR cylinder to insure proper operation of the replaced fuel i pump. Following successful completion of the maintenance run, the 1A DG was started per the monthly operability surveillance. At 2357, on September 25, 1987 the operability surveillance was completed and If. OAR 1BOS 8.1.1-la was exited.

BYRON 87-1162 Page 2 The root cause for this event was a cracked fitting at the SR cylinder fuel pump discharge. The fuel pump has been sent to the Commonwealth Edison Material Analysis Department for a failed component inspection.

I The 1A diesel generator was inoperable for 9 hours1.041667e-4 days <br />0.0025 hours <br />1.488095e-5 weeks <br />3.4245e-6 months <br /> and 22 minutes as a result of this even+.. This failure was the second failure during the last 20 valid I tests on the 1A DG, therefore, per table 4.8-1 of the Byron Technical Specifications, the 3A DG will be placed on a weekly testing frequency.

This failure was the seventh failure during the previous 100 valid tests for Unit 1, therefore, this report is supplemented with the additional information requested by Regulatory Guide 1.108 C.3.b. Attachment A lists the summary of all tests, valid and invalid, that occurred within the time period over which the last 100 valid tests were performed.

Several corrective actions have been taken, and are planned, to increase the reliability of the Unit 1 diesel generators based on tne failures experienced. Four of the seven failures were caused by the engine not reaching rated speed, voltage, and frequency within the required ten seconds.

Byron Station believes that a major contributor to these failurec is a loss of fuel prime. To correct this problem, soft seat check valves were added in the fuel oil supply line to prevent back leakage of the fuel cil. In addition a modification is currently in progress to add a loop seal and an anti-syphon vent to the fuel supply line. These changes will guarantee that the engine will have an adequate supply of fuel oil during the start sequence.

1 One failure was caused by degradation of a relay which transfers an electronic signal to the electronic governor. Because of the type of signal transferred through tne relay, minor rasistance can cause governor oscillations. A modification has been initiated to replace the subject relay with a relay that has lower contact resistance.

The other two failures which occurred during the last 100 valid tests include the failure addressed in this repcrt, and the failure of the 1A diesel generator stator on November 23, 1985. The root cause for the stator fcilure was a loose nut which was found inside the stator housing. The rut apparently was thrown into the windings from the rotor fan blades. The origin of the nut was never determined. All maintenance departments were reminded of the importance of thorough inspectior.s of rotating equipment prior to and during reassembly. No failures of this type have occurred since the November 23, 1985 failure.

Byron Station has taken comprehensive corrective actions to rectify the current diesel generator problems.

BYROM 87-1162 Page 3 Byron Station believes that the overall reliability of electric power to Engineered Safety Feature equipment is adequate. The primary source of offsite power, the Unit 1 system auxiliary transformers and associated breakers, has experienced 100% reliability since receipt of the Unit 1 operating license. The backup source of offsite power, the Unit 2 system auxiliary transformers and associated breakers, has experienced three unrelated failures during this same period. In all cases the Unit 2 diesel generators operated per design. The reliability of offsite power and the associated transformers and breakers, in sddition to the confidence Byron Statior has in the future operction of the emergency diesel generators, provides ths basis for cont'.nued plant operation.
