Diesel' Generator Failure Report

Dear Mr. Davis:

This Report is submitted in accordance with Byron Station Unit 2 Operating License NPF-66,' Appendix A, Technical Specification 4.8.1.1.3, reporting of Diesel Generator failures.

This report addresses one nonvalid test and failure experienced on the 2B Diesel Generator. The criteria to determine valid tests and failures is ir, accordance'with Section C.2.e of Regulatory Guide 1.108.

On February.14, 1989, the 2B Diesel Generator was started in accordance with the monthly operability surveillance, 2BOS 8.1.1.2.a-2. Following a successful start and warmup period, a Nuclear Station Operator (NSO - licensed) attempted to load the Diesel Generator to the grid. When the NSO closed the output breaker he failed to apply adequate initial load to the Diesel Generator.. Because of this inadequate loading, the 2B Diesel Generator tripped on " REVERSE POWER".

At 1635 on 2/14/89, the 2B Diesel Generator was restarted in accordance with the monthly operability surveillance. At 1825 on 02/14/B0, the surveillance was successfully completed.

  • , ' BYRON 89-0277 Page 3 The root cause for this event was a combination of a procedural discrepancy,and inexperience on the NSO's part. The procedure which directs the paralleling of

.the Diesel Generators, BOP DG-11, does not direct the NSO to 1mmediately apply load to the DG after the output breaker is closed. Ceneral training and experience has taught the NSO's of this requirement, however, the NSO who was involved had limited experience in loading the Diesel Generators.

Corrective actions for this event included counseling the NSO involved in tne event, and initiating a procedure change to BOP DG-11 to direct immediate loading of the Diesel Generators following closure of 'Se output breaker.

Action Item Record 454-255-89-0086 will insure completion of the procedure change.

The 2B Diesel Generator.was not required to be operable at the time of the event due to plant conditions, therefore the diesel was not considered inoperable for any period of time. In accordance with Section C.2.e.(2) of Regulatory Guide 1.108 this event has been considered a nonvalid failure, therefore the testing frequency for the 2B Diesel Generator was not affected by this event.
