p-o I ATTACHMENT A COMMONWEALTH EDISON COMPANY BYRON NUCLEAR POWER STATION DOCKET 50-454 MATERIALS LICENSE NO. SNM-1917 DOCKET 70-2973 Description of Event On June 30,1983, the Byron Station Nuclear Engineer and a Fuels System Test Engineer were inspecting a spent fuel rack in the Spent Fuel Pool. At approximately 1830 hours0.0212 days <br />0.508 hours <br />0.00303 weeks <br />6.96315e-4 months <br />, as they were leaving the area, the Security Guard on duty at the New Fuel Storage Vault (NFSV) informed them of five propane gas cylinders in the area. The cylinders were observed tied to a guard rail at the south end of the Spent Fuel Pool.

The Auxiliary Building night supervisor was instructed to remove the five cylinders from the Fuel Handling Building (FHB). All cylinders were removed from the FilB by 2000 hours0.0231 days <br />0.556 hours <br />0.00331 weeks <br />7.61e-4 months <br />.

Consequence of Occurrence The five propane gas cylinders represented a heat load in excess of that analyzed for the pre-fire plan of the FHB. This was a violation of a condition authorizing the receipt and storage of nuclear fuel. Subsequent investigation revealed the cylinders had been moved into the building at 1500 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.7075e-4 months <br />, for an overall duration of approximately five hours. At the time of the violation 86 fuel assemblies were stored in the NFSV and 10 assemblies were in 5 shipping containers stored on the 411 foot elevation platform.

Cause of Occurrence The propane gas was required for a steam generator needed for cleaning the Spent Fuel Poolliner and fuel storage racks. Contractor personnel responsible for this activity were unaware of the combustible materiallimitations in effect for the FHB.

Corrective Actions The propane gas cylinders will be stored outside the FHB and gas delivered to the steam generators through quick closure valves.

To prevent further violation of the pre-fire plan, a written instruction directed the 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> Security Guard Force to control entry of combustible material into the secured area of the FIIB. Project Construction Department was notified of the violation and the limitations in effect on combustible materials in the FliB. Additionally, signs will be posted to warn contractor personnel relative to combustible material controls. This measure will be achieved by July 25,1983.
