SUBJECT:
POLICY FOR THE USE OF FORM NRC-591 The NRC-591 form provides a convenient method of citing violations of minor safety significance and the use of this form in the field reduces the backlog of routine enforcement correspondence.
Therefore, you should report inspection results on Form NRC-591 whenever possible.
However, more significant violations require correspondence between the NRC and the licensee to document the licensee's proposed corrective. actions and to permit NRC review of these actions.
The Attachment to this memorandum provides a list of violations which may be cited during inspections using Form NRC-591..These include all Severity Level V violations and some Severity Level IV violations.
A Form NRC-591 should not be used when there are more than three violations, when there is a recurrent violation, when there are safety concerns that are not violations, when a formal report will be issued, or for inspections which involve the review of multiple diagnostic misadministrations or a single therapeutic misadministration.
While you should issue a 591 whenever possible, you may recommend issuance of a formal Notice of Violation to your Section Chief in any circumstance when you think this action is necessary.

O VIOLATIONS THAT MAY BE CITED ON FORM NRC-591 1.
Violations involving:
a.
The posting of " Caution, Radiation Areas" or "
Caution Radioactive Material" storage areas.
b.
The labelling containers of radioactive material, except when associated with medical misadministrations.
c.
Leak testing of sealed sources.
d.
Records of required surveys or other recordkeeping violations.
e.
Posting required by 10 CFR 19.11.
f.
Reporting required by 10 CFR 20.407(b),
10 CFR 20.408(b), and 10 CFR 20.409(b).
g.
Calibration of survey meters and pocket dosimeters.
h.
Inventory requirements.
1.
Notification of NRC when authorized user, RSO or teletherapy physicist discontinues the performance of activities.
J.
Eating, drinking, etc. where radioactive materials are use.
2.
Any Severity level V violation.
