O'Reilly LER No.82-033 Page Two November 29, 1982 DETAILED DESCRIPTION'OF EVENT On October 31, 1982, with the Plant in Mode 4, while performing Surveillance Test Procedure (STP) 123.002, " Service Water System Pump Test", the Speed Switch for Service Water Pump "A" failed in mid-position during the fast to slow speed transfer. Service Water Train "A" was declared inoperable per Technical Specification 3.7.4.

PROBABLE CONSEQUENCES Since the redundant Service Water Train "B" -was operable at the time of this event, there were no adverse consequences.

CAUSE(S) OF THE OCCURRENCE The cause of this' occurrence was determined to be a bad contact on the Speed Switch Relay. The charging motor consequently failed to adequately charge the springs. causing the Speed Switch to shift only to mid-position.

IMMEDIATE CORRECTIVE ACTIONS TAKEN Speed Switch (XES-2003A) was manually shifted to fast speed and Service Water Pump "A" was started within fifteen (15). minutes of the occurrence. Service Water Pump "A" was removed from service af ter completion of Service Water Pump "C" alignment to Train "A". Speed Switch Relay repair was performed satisfactorily on November 1, 1982.

i ACTidd TAKEN TO PREVENT RECURRENCE Speed Switch Relay inspection has been added to the Station Preventive Maintenance Program to prevent recurrences of this nature.
