FED .f ,,3 p g-!'. ) Gentlemen: O g sc.- The purocse of this letter is to report three ceparate occasions when the vent valve on the spent fuel shipping casks used at San Onofre Nuclear Generating / Station, Unit 1, was found open upon arrival at the General Electric Spent Fuel Storage Facility in Morris, Illinois. Submittal is in accordance with direction received from the Office of Inspection and Enforcenent, Region V, and the pro-visions of 10CFR 71.61. Identifying information on the three seoarate shipnents, where the vent valve was found to be open on arrival at the General E.lectric Spent Fuel Storage Facility, is listed in Table 1. Three different casks with the following 16enti-fication nu.Tbers are currently used to ship spent fuel at San unofre: NAC-IS, i&C-lC, HAC-1D. Station precedures used in preparing the casks for shiperent of spent fuel require that the vent valve be c1cse6 before offsite chip::ent. Check-off sheets associated with these procedures are used to document the fact that the vent valve is closed before off-site shioment of each cask. The cm.pleted check-off sheets for the three shiprents cited above show the vent valves to have been closed prior to shipacnt. ] As shown in the attached Figure 1, the vent valve is located on a 3/4" I.D. pipe which originates in the spent fuel cask cavity. Downstream of the vent valve (reference figure 2) a 1/2" quick disconnect cnap-tite valved coupler is attached to act as a hose c1crp when flushing the cosk cavity. A cover plate I with a gasket is used to ceal the recessed cavity containing the vent valve and encp-tite. Tne co::ponents of the vent valve are shown in ficure 3. Although the vent valve is considered a primary boundary, no significant contmination would be expected with the valve ooen. The presence of the sc.mstrea::: cnap-tite valved coupler, and the coverplate with gasket, would at 1 cast ninimize corrunication of the cask cavity with the environs. p t 1 . An examination of the vent valves has suggested that the packing nay not have been sufficiently tightened. Eith a loosened packing, vibration during transit n.ay have caused the valves to cren. Since adjustrent of the packings, all casks have arrived at the General Electric Spent Fuel Storage Facility with the vent valves in the closed position. Spent fuel shipping procedures new recuire that the condition of the vent valve be ascertained prior to releasing the cask for shipment. Adjustrents are rade as necessary to assure that the packing is sufficiently tightened to n.iniraize the possibility of the valve roving to the open position during transit.
