" Simulated Auto Initiation of Diesel Generators", Core Spray Pump P-215B failed to start and Core Spray valve 1400-24B failed to automatically open. On the same day, following a procedure change, the test was repeated with the Core Spray Pump automatically starting. On the following day valve 1400-24B opening -

circuit was adjusted by Station I&C personnel, tested and returned to service.

At the time of the test neither the core spray nor the LPCI systems were required to be operable.

Cause and Corrective Action Core Spray Pump P215B did not start because its breaker number 152-607 ex-perienced a trip free operation. The trip free operation resulted from a timing sequence in which the test initiating signals play a vital part. A procedure change was made to procedure number 8.M.3-1 prior to the test being repeated.

The procedure change, which instructed that an existing test switch in the logic circuit be utilized, altered the logic circuits timing and eliminated the trip free operation during the test. A plant design change request number 77-115 is addressing a permanent resolution to this problem. A detailed report on this trip free operation was previously issued on December 12, 1977 in LER 77-40.

During the performance of the test valve 1400-24B did not open. The auto-matic opening of this core spray discharge valve was prevented by a dirty contact in its final actuating relay, 14A-K13B. The relay is a G.E. Model No.

12HEA51A42F. The contact was cleaned, the relay and valve operationally tested and the valve returned to service. As a result of this event, a review of all similar ECCS circuits is being performed to ensure that the final actuating contacts in valve logic circuits are periodically tested and capable of performing their intended function.
