The Control Operator instructed the Auxiliary Operator to hang a clearance on the "B" Core Spray pump for pump maintenance. The Auxiliary Operator was informed that all " motor operated" valves were in their required clearance positions.

The Auxiliary Operator mistook this to mean that all valves, except the pump vent and drain valves, were in their required positions and did not check the valve positions when hanging the cicarance tags.

The

. Auxiliary Operator had not completed the on-the-job training check-off sheets which indicate clearance training.

Shift Operating Supervisors have reviewed the training records of all personnel assigned to their shifts to ascertain those qualified to hang clearances and ensure that only those personnel are allowed to hang clearances. TI-104 will be revised to require that sections involving clearances be completed prior to the operator being qualified to hang clearances.

This LER will be reviewed by all Operations personnel.
