Southwest Research Institute (SWRI) had maintained the weld inspection portion of Salem Units 1 and 2 ISI program during the first 10 year intervals. A review of Salem Unit 2 ISI first 10 year interval welds revealed one additional longitudinal weld on the Pressurizer that had not been completed.

Non performance of these ISI weld inspections constitutes a missed surveillance required by Technical Specification 4. 0. 5.

The cause of the missed ISI of the two welds is insufficient verification and validation of the entry of data into the SWRI ISI database. Corrective actions include completed examination of the Pressurizer Spray Nozzle and Pressurizer longitudinal welds with acceptable results. The Salem Unit 1 first 10 year ISI required welds will be reviewed and any identified missed surveillances will be completed by entry into Mode 6 for Salem Unit 1.

This event is reportable in accordance with 10 CFR 73(a) ( 2) {i) ( B) ' any condition prohibited by the plant's Technical Specifications.

CONDITIONS PRIOR TO OCCURRENCE At the time of identification, Salem Units 1 and 2 were shutdown and defueled.

DESCRIPTION OF OCCURRENCE On August 19, 1996 a determination was made that the first interval Inservice Inspection (ISI) for the Pressurizer spray nozzle inner radius had not been performed. Southwest Research Institute (SWRI) had maintained the weld inspection portion of Salem Units 1 and 2 ISI program during the first 10 year intervals. A review of Salem Unit 2 rsr first 10 year interval welds revealed one additional longitudinal weld on the Pressurizer that had not been completed.

Non performance of these ISI weld inspections constitutes a missed surveillance required by Technical Specification 4.0.5.

A PSE&G review of the ISI first interval Non-Destructive (NOE) ISI records for these welds could not locate data that the inspections were performed. A similar check for these two welds was also conducted by SWRI with the same results.

SWRI had utilized a PC database to document the weld examination status. The PC program required manual entry of data into the database. This entry .was typically performed at the SWRI home office after the outages during which the ISI weld inspections were completed. Starting with the second ten year ISI interval for Salem Unit 2, which commenced in 1992, PSE&G assumed control for the implementation of the entire Salem Unit 2 ISI program. PSE&G assumed control for Salem Unit 1 ISI program in 1993. PSE&G does not use the SWRI database in Salem Unit 1 or 2 ISI programs. The current PC database has inherent program controls linking completed NOE examinations with exam schedule updates and final reports.

CAUSE OF OCCURRENCE The cause of the missed ISI of the two welds is insufficient verification and validation of the entry of data into the SWRI ISI database. Insufficient controls were in place to ensure that correct entries were made.

PRIOR SIMILAR OCCURRENCES A review of LERs for Salem Units 1 and 2 over the last two years did not identify any similar issue.

SAFETY CONSEQUENCES AND IMPLICATIONS There were no safety consequences with this event because the welds have been examined and found acceptable. If the recent examination of the welds had not been satisfactory, the implication is that indications in the welds could have developed into cracks. With cracks in the welds, small leaks could have occurred that would result in a loss of Reactor Coolant System (RCS) inventory.

It is reasonable to expect that the leaks would have been detected and the loss of RCS inventory would be recognized, with appropriate operator action being taken.

CORRECTIVE ACTION

1. The Salem Unit 2 first 10 year ISI required welds were reviewed to ensure no other welds were missed.
2. Examination of the Pressurizer Spray Nozzle and Pressurizer longitudinal welds were completed with acceptable results.
3. The Salem Unit 1 first 10 year ISI required welds will be reviewed and any identified missed surveillances will be completed by entry into Mode 6 for Salem Unit 1.
