LICENSE NO. DPR-70 DOCKET NO. 50-272 REPORTABLE OCCURRENCE 8l-30/03L Pursuant to the requirements of Salem Generating Station Unit No. 1 Technical Specifications, Section 6.9.1, we are submitting Licensee Event Report for Reportable Occurrence 81-30/03L.

This report is required within thirty {30) days of the occurrence.

Although the report was scheduled for review and transmittal during the week of April 6, unusual station activities during this week and post-ponement of the scheduled Station Operating Review Committee meeting resulted in the late. submittal of this report.

Non-f\\lnctional Fire Barriers CONDITIONS PRIOR TO OCCURRENCE:

Unit 1 - Various operational modes Unit 2 - Mode 5 DESCRIPTION OF OCCURRENCE:

At approximately 0900 hours0.0104 days <br />0.25 hours <br />0.00149 weeks <br />3.4245e-4 months <br /> on March 12, 1981; the Senior Shift.

Supervisor on duty was requested by maintenance personnel to grant permission to replug several penetration fire barriers (3-1/2" Welding Lead Plugs).

At that time, the Senior Shift Supervisor*

discovered that no fire watches had been established when the plugs were removed on February. 18, 1981 in accordance with Action Statement 3.7.11 as the Action Statement had never been entered or.

noted in any of the operations logs.

The Welding Lead Plugs had been removed in order to runhoses from the.Unit 2 Component Cooling Water Surge Tank to a portable demineralizer tank van parked behind the Rad Waste Area*and from the van to a manifold in the Component Cooling Water Heat Exchanger Room.

The hoses were necessary to provide a parallel flow. path while. dechromating the Component Cooling Water System.

The following Welding Lead Plugs were removed:

two (21 from the area of the elevator lobby on the 84' Elevation in the Auxiliary Building~ two (2) from the area of the Solid Rad Waste Penetration Door on the 100' Elevation in the Auxiliary Building~ one (1) from the area of the elevator lobby on the 122' Elevation in the Auxiliary Building.

The Senior Shift Supervisor directed that a late entry be made in the log and established.the necessary fire watches.

on March 13, 1981, all the* fire barriers had been replaced.

This occurrence constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b.

81-03/L 4/15/81 DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

  • Personnel Error.

Personnel involved in the removing of the plugs

  • and the running of the hoses were not familiar with the procedures and documentation required to perform this task.

ANALYSIS OF OCCURRENCE:

The 3-1/2" Welding Lead Plugs are designed to retard the heat transferred from a fire on one side of the penetration to the other for a period of three (3) hours.

Technical Specification 3.7.11 requires that with one or more of the penetration *fire barriers non-functional, a continuous fire watch be established on at least one side of the* affected penetration within one hour.

CORRECTIVE ACTION

The personnel involved in this occurrence have been instructed concerning the existing procedures for fire barriers.

To establish administrative control to ensure that appropriate personnel are notified in the event that a fire barrier or a security boundary is going to be breached, Administrative Procedure AP-9, Work Order System, is currently undergoing revision.
