IDENTIFICATION OF OCCURRENCE:

Failure to log and acknowledge operational events with entries into Action Statements

' CONDITIONS P~IOR TO OCCURRENCE:

During a routine inspection on January 1, 1981 - January 31, 1981, the NRC Resident Inspector discovered the following failures to log and acknowledge entry into an Action* Statement.

On January 7 and 8, *1981, Secondary Plant Log readings recorded Nmnber l Fire Pump Day Tank level as less than that required

No corrective action or acknowledgemen.t. of an Action Statement was identified.

Tank level was restored within the time limits dictated by Technical

. Specifications.

On January 20, 1981,.Au.xiliary Feedwater Storage Tank Level

.was noted as 86%, which is less than the 94% specified by Technical Specification 3.. 7.1.3.

No corrective action had been initiated *or ackn.owledgement of the Actiori Statement made.

The tank was refilled within the. time limits dictated by Technical Specifications.*

3. On January 27, 1981, Pressurizer Overpressure Protection Channel 2 on Unit 2 was not in service.

No reason or acknow-.

ledgement of failure to have two POPS channels in service, as required by Technical Specification 3.4.9.3, was identified.

The channel was restored to service within the time limits

. specified in the Action Statement.

Each of the above discoveries constituted operation in a degraded mode in accordance with Technical Specification 6.9.1.9.b *

. DESIGNATION.OF :APPARENT CAUSE OF OCCURRENCE:

Personnel Error - In each case, Operations personnel neglected.to make the appropriate log entries.

4/16/81 ANALYSIS OF OCCURRENCE:

The deficiencies noted by the Resident NRC Inspector indicate the need for formal training sessions in the operator requal:'j:fication program that will address the importance of accurate and timely documentation of operational events in the various station logs.

CORRECTIVE ACTION

The need to enter Action Statements has been emphasized upon the operators.

In the next operator requalification training cycle, which will start in September 1981, lectures will be added to the-course that will address the importance of-accurate and timely documentation of operational events in the various station logs.
