Loss of Level Indication on Nos. 11 and 12 Boric Acid Tanks CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 -

Rx Power 100% - Unit Load 1120 MW DESCRIPTION OF OCCURRENCE:

During normal operations at 1600 hours0.0185 days <br />0.444 hours <br />0.00265 weeks <br />6.088e-4 months <br /> on February 27, 1981, Nos. 11 and 12 Boric Acid Tanks were declared inoperable because of the loss of level indication for both tanks.

Action Statement 3.l.2.8a was entered at this time.

This occurrence constituted operation in a degraded mode IAW TS 6.9.l.9b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The sensing lines of both level transmitters had become plugged with boric acid.

ANALYSIS OF OCCURRENCE:

Technical Specifications require that with the Boric Acid Storage System inoperable, restore the storage system to operable status within the next 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in at least hot standby within the next six hours and borated to a shutdown margin equivalent to at least 1% 6 K/K at 200°F; restore the Boric Acid Storage System to operable status within the next seven days or be in cold shutdown within the next 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.

CORRECTIVE ACTION

Both transmitter sensing lines were cleaned by blowing them down with pressurized nitrogen gas.

The Action Statement was terminated at 1658 hours0.0192 days <br />0.461 hours <br />0.00274 weeks <br />6.30869e-4 months <br />.
