No. 12 Centrifugal Charging Pump CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 -

Rx Power 25% - Unit Load 260 MW DESCRIPTION OF OCCURRENCE:

During normal operation on February 1, 1981, the No. 12 Centrifugal Charging Pump was declared inoperable because of oil leakage.

Action Statement 3.5.2.a was entered at 1700 hours0.0197 days <br />0.472 hours <br />0.00281 weeks <br />6.4685e-4 months <br /> which requires that an inoperable ECCS subsystem must be made operable within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

This occurrence con-stituted operation in a degraded mode IAW Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure Oil seals were leaking.

ANALYSIS OF OCCURRENCE:

Loss of oil caused the thrust bearing to overheat and the main bearings to partially wipe.

CORRECTIVE ACTION

The main bearings and thrust bearings were replaced.

All oil lines were cleaned and flushed.

The oil reservoir and all bearing housings were also cleaned and flushed.

A satisfactory surveillance test was performed and the Action Statement was terminated at 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br /> on February 3, 1981.
