Turbine Impulse Channel Functional Test Failure (Comparator 1FC513 Failed)

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 -

Rx Power 63% - Unit Load 700 MW DESCRIPTION OF OCCURRENCE:

During normal operations at 0820 hours0.00949 days <br />0.228 hours <br />0.00136 weeks <br />3.1201e-4 months <br /> on January 28, 1981, while performing a routine functional test on the Reactor Protection System (High Steam Flow, Channel II for Safety Injection), the Safety Injection signal could not be generated.

IAW Technical Specification 3.3.2.b normal operation continued with the inoperable channel placed in the tripped condition.

A work order was issued to trouble-shoot and repair the channel.

This occurrence constituted operation in a degraded mode IAW Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

Equipment Failure ANALYSIS OF OCCURRENCE:

Upon troubleshooting the channel, it was determined that the Comparator Unit (Hagan Model 4111082-001) had failed.

CORRECTIVE ACTION

The defective Hagan Module was replaced with a spare.

The replace-ment unit was calibrated and a satisfactory channel functional test was performed.

The channel was declared operational at 1050 hours0.0122 days <br />0.292 hours <br />0.00174 weeks <br />3.99525e-4 months <br /> on January 28, 1981.
