Loss of Audible Fire Detection Signals CONDITIONS PRIOR TO OCCURRENCE:

Rx Power 0% - Unit Load 0 MW DESCRIPTION OF OCCURRENCE:

During normal operations at 0950 hours0.011 days <br />0.264 hours <br />0.00157 weeks <br />3.61475e-4 months <br /> on January 15, 1981, the operating staffs of Units 1 and 2 were notified by the PSE&G Startup Group that the Fire Protection System Tone Generators were inoperable and that no audible coded fire signals would sound.

Action State-ment 3.3.3.6 was initiated at this time.

An operator was stationed at the fire alarm panel in the Control Room to continually monitor the panel for visual fire alarm signals.

This occurrence constituted operation in a degraded mode IAW Technical Specification 6.9.1.9.b.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

An impedance mismatch was* created between the station phone system, the station page system and the tone generator when additional page_ extension phones and rotating fire alarm beacons were added to tone generator's electrical load.

ANALYSIS OF OCCURRENCE:

Two years ago Gaitronics (vendor) personnel balanced the entire system.

The probable unbalancing of the system was not considered when the additional loads were added.

CORRECTIVE ACTION

The vox-Anux amplifier in the tone generator unit was adjusted to obtain the correct impedance matching between the the tone generator and the remainder of the system.

In the future, as new loads are added to the system, readjustment of the amplifier will be included in the installation instructions.

The Action Statement was terminated at 1225 hours0.0142 days <br />0.34 hours <br />0.00203 weeks <br />4.661125e-4 months <br /> on January 15, 1981.
