Valve 12 SW124 was tagged out at 0730 hours0.00845 days <br />0.203 hours <br />0.00121 weeks <br />2.77765e-4 months <br /> January 9, *1981 DESCRIPTION OF OCCURRENCE:

During normal operations at 0730 hours0.00845 days <br />0.203 hours <br />0.00121 weeks <br />2.77765e-4 months <br /> ori January 9, 1981, Service Water Valve l2SW124 (Drain Valve on No. 12. CCWHX) was tagged out for a leaking connecting nipple.

At 0800 hours0.00926 days <br />0.222 hours <br />0.00132 weeks <br />3.044e-4 months <br />, the No. 12 CCWHX was tagged out of service leaving only one Component Cooling Water Loop operable.

Action Statement 3.7.3.l was* entered at this time.

This occurrence resulted irt operation in* a. degraded* mode IAW Technical Specification 6. 9. l. 9. b.

DESIGNATION OF. APPARENT CAUSE OF OCCURRENCE:.

The weld between the Heat Exchanger Tube Sheet *and the'connecting nipple for Valve l2SW124 had failed.

ANALYSIS.OF OCCURRENCE:

The Heat Exchanger Tube Sheet is made of Monel metal. *The nipple is carbon steel.

The leak was found to be at the weld bonding these dissimilar metals.

CORRECTIVE ACTION

The original.nipple was replaced and the welded connection completely r*eworked.

The original valve was reinstalled.

A Design Change Request has been initiated which will replace the carbon steei nipple with one made of Monel metal which will allow a better fusion of the weld.

The Action Statement was terminated at 1400 hours0.0162 days <br />0.389 hours <br />0.00231 weeks <br />5.327e-4 months <br /> on January.11, 1981.
