12 Charging Pump Inoperable CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 100%

DESCRIPTION OF OCCURRENCE:

During normal operation, it was observed that there was service water in the lube oil reservoir.

At 0100 hours0.00116 days <br />0.0278 hours <br />1.653439e-4 weeks <br />3.805e-5 months <br />, the pump was declared inoperable and Action Statement 3.5.2a was entered requiring, with one ECCS subsystem inoperable, to restore the inoperable subsystem to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in hot shutdown within the next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

A tube failure in the lube oil cooler.

ANALYSIS OF OCCURRENCE:

Investigation revealed there was a tube failure in the lube oil cooler resulting in a service water to lube oil leak.

CORRECTIVE ACTION

The cooler was replaced in kind with one from stock, tested satis-factorily and the Action Statement was terminated at 1445 hours0.0167 days <br />0.401 hours <br />0.00239 weeks <br />5.498225e-4 months <br /> on June 27, 1980.
