03/18/16 Observation or Comment:

Prior to performing the GWS of LSA 08-12, a quality walk down of the area was conducted. Isolation controls consisting of green and white rope were in place. Asphalt and concrete pieces were removed from the area and disposed of properly before the GWS was performed. Weather conditions were cool and overcast at 51 degrees Fahrenheit. Ludlum 2221 and 44-10 B and N were used to perform the GWS. The background for instrument B was 10,500 GCPM. The background for instrument N was 10,000. The GWS was started at 0900 and paused at 1055. The GWS resumed at 1300 and was completed at 1530. LSA 08-12 was exited and the personnel returned to the office to process data and perform instrument post-checks.

Prior to commencing the biased sampling of LSA 08-12 the FSS Supervisor conducted a task-specific safety brief. The FSS personnel performed a quality walk-down to ensure I&C measures were in place. Weather conditions were clear and cool at 63°F. Biased sampling commenced at 1555 and was secured at 1635. The FSS personnel returned to the FSS office to prepare samples for shipment.

The FSS personnel performed a quality walk-down to ensure I&C measures were in place.

Weather conditions were clear and cool at 63°F. General area background was measured at 8K-10K. The GWS commenced at 1349 and was secured at 1501. A second GWS was conducted from 1615 to 1621. Biased sampling commenced at 1620 and concluded at 1630. LSA 08-12 was exited and the personnel returned to the office to prepare samples for shipment, process data and perform instrument post-checks.
