Enclosure to ULNRC04956 March 4, 2004 Page I of I ENCLOSURE, Special Report The Loose-Part Detection System Limiting Condition for Operation (LCO) (Section 16.3.3.5 of FSAR) requires the loose-part detection system to be operable in Modes 1 and 2. With one or more Loose-Part Detection System channels inoperable for more than 30 days, LCO Action 'a' requires a Special Report to be submitted to the Commission within the next 10 days outlining the cause of the malfunction and the plans for restoring the channel(s) to operable status.
On January 27, 2004, with Unit in Mode 3, Channel 12 of the LPDS was declared inoperable. This is one of the two sensors that mounted on the inlet plenum side of Steam Generator "D".
Cause of the malfunction:
The initial investigation has concluded that the failure is likely associated with the channel components or cable connections located inside the primary containment, as indicated by the channel's excessive noise and a constant hum on its audio output.
Plans for restoring the channel to Operable status:
The capability to detect loose metallic parts in the Reactor Coolant System has been retained with the remaining 11 operable channels. The redundant sensor at the Steam Generator "D" inlet plenum is operable. Therefore continued operation to Refuel 13 is acceptable.
A station Work Request has been written to repair Channel 12 of the LPDS. The repair and testing to return the channel to operable status will be scheduled for the next refueling outage (RFO 13), due to limited accessibility resulting from high radiation dose rates during normal plant operation. Refuel 13 is currently scheduled to begin on April 10, 2004.
Repairs will be completed prior to restart from Refuel 13.
COMMITMENT DESIGNATION PROCESS LIST OF COMMITMENTS The following table identifies those actions committed to by Callaway Plant in this document. Any other statements in this submittal are provided for information purposes and are not considered to be commitments. Please direct questions regarding these commitments to Mr. Mark A. Reidmeyer, Regional Regulatory Affairs Supervisor (573) 676-4306.
COMMITMENT Due Date/El vent Restoration of Channel 12 of the Loose Parts Monitoring Prior to restart from Refuel 13 System to Operable Status Note: Refuel 13 is currently scheduled to start April 10, 2004.
