Resident and Reactor Project Inspection SUf1 MARY Inspectior. on September 1-30, 1981 Areas Inspected This routine inspection involved 73 inspector-hours on site in the areas of structural concrete, A-2 steel containment welding, earthwork, open items, independent inspection effort, observation of nondestructive examination (Unit A-1), safety-related structural work activities and nondestructive examination (Unit A-1) and review of NDE implementing procedures.

Exit Interview The inspection scope and findings were summarized with the Site Project flanager on September 4, 18 and October 1, 1981.

Licensee Action on Previous Inspection Findings (Closed) IFI 519/80-01-01 IIT Indications on Anchor Darling Globe Valve NCR HilPB-033 QCIR 26082 Site TVA memorandum dated September 9,1981, entitled Hartsville Nuclear Plant - Evaluation of the NDE of the Valve Body and Associated Repairs or Anchor Darling Valve Serial No. E-6318-40-8 per NCR B-033, d1 tails the findings of a TVA level 3 inspector, J. W. Henry, Supervisor Welding Quality Control, that this valve is unacceptable.

iiCR paragraph 9 notation dated 9/25/81 states that the valve is unacceptable and is to be returned to vendor. This action was approved by the construction engineer.

TVA has committed to notify the resident inspector when the valve is returned to vendor or scrappe.

Indepdent Inspection Effort

The senior resident inspector made 5 surveillance tours during.which the status of project work was noted, and construction activities were inspected en nuclear safety-related structures, systems and facilities. The inspection was concentrated on' Plant A power block structures, but included outlying structures such as Plant A essential service water pum,n stations, earthfill for the A-1 and A-2 spray ponds, the intake pumping station, and circulating cooling water pump house.

Open Items. Control: Newly issued Nonconformance Reports (NCRs), significant i

Quality Control Inspection Reports (QCIRs) and QA Unit audit findings were reviewed for pertinence to the inspection program and for adequacy of corrective actions. The review of CDR's and NCR's did not disclose any violations or unresolved items.

Observation of welding activities within the containment, steel structures:

The resident inspector observed in progress welding activities to the A-2 containment cylinder wall to determine compliance with applicable code and

piocedure requirements.

The code applicable to the containment vessel is

the ASME Boiler and Pressure Vessel Code,Section III, Sub section NE class MC 1974 Edition, including addenda through S74.

Welds observed in progress identified as A2CS00419 and A2CS00421 performed by qualified welders numbers BADA and BAFC respectively. Weld procedure specification (WPS) SM-11-B9 Revision 4 was applicable. The ir.spector observed welders' technique, fit-up and geometry, electrical cha acter-istics, preheat and interpass temperature, weld rod size, type and humidity control for low hydrogen type electrode.

Within the area inspected no violations or deviations were identified.

Lakes, Dams, and Canals--Review of Work and Work Activities--Earth Fill For Plant A Spray Ponds The inspector made random inspections of earth placement, compaction ~and compaction testing on the earth work for Plant A essential service water spray ponds. Compaction testing in the field was inspected. Whenever the result was unacceptable, the sampled area was reworked and retested until adequate compaction was obtained.

Placement and compaction of earth fill for the peripheral dikes around the spray ponds is controlled by Specification N6C-875 Earth and Rock Founda-tions and Fills. By observation of placement and testing operations, the i

inspector verified compliance with Specification N6C-875 and with quality control procedure QCI C-102, Foundation and Fill, Earthfill Control Testing.

In the areas inspected, no violations or deviations were identified.

8.

Seismic Category I Concrete Forming, installation of rebar, positioning of embedments, placements and post placement curing were inspected as work progressed in the Plant A fuel buildings, auxiliary buildings, control buildings, essential service water pump houses, and intake pumping station. Specific placements inspected included pour AIA 68B totaling 90 cubic yards in A-1 auxiliary building walls, AIF-40C and A2F 29B (62 CY).

The walls for the steam tunnel in A-1 auxiliary building were complete and were inspecte _.

Inspections were made of the installation of rebar for the A-2 weir wall and for the 12-foot thick floor pad between the A-2 RPV pedestal and the weir wall. The completed stainless steel liner for the weir wall was inspected.

During inspection of activities.for structural concrete in these areas, the-conformance with the requirements of C. F. Braun Specification 300-0, Revision 8, " Concrete", TVA Construction Specification G-2, QCl C-201, Rev.

4 and CEP 9.02, Revision 5, was verified by the inspector.

In the areas inspected, no violations or-deviations were identified.

Work Activities Outside of Containment The resident inspector reviewed receipt inspection records for structural

steel to be utilized in safety-related areas.

Records were reviewed for

completion of receiving inspection check list (TVA Form 10152) in accordance

with TVA's Receiving, Storage and Preventive liaintenance Inspection llanual,

including verification of manufacturers' documentation, such as certified material test reports, vendor QC reports, and TVA inspection shipping release.

The inspector selected a random sample of structural steel components

currently in storage and installed. The following contract and serial

Work package number C-154-C was reviewed for references of applicable

procedure and specification requirements.

The inspector verified

inspections of line and grade of erected beams, and bolted connections for i

proper hardware and installation. Welded connections were visually examined l

and required nondestructive examinations (NDE) were spot checked.

Radio-graphs (RT) of butt welds identified as AIFB STR STL.25 and 26 were l

reviewed.

However, the inspector could not determine the acceptance criteria utilized from the radiograph report sheet, as no radiograph procedure was specified.

This discrepancy was brought to the attention of cognizant licensee personnel who informed the inspector that RT procedure QCI-N-302 Revision 2 was applicable. The importance of identification of applicable procedures on NDE reports was discussed with NDE supervision. Licensee's NDE super-vision investigated the omission of the procedure identification and concluded the radiograph data report form as illustrated in QCI-N-302 Revision 2, had inadvertantly left off the procedure number. This was apparent to the inspector as two forms are optional in the procedure, one has the procedure number printed on the form, the other does not. The licensee contends they had maintained traceability of the R. T. procedure utilized in the Work pckage, but agreed to revise the radiograph report form to include the applicable radiograph procedure identification.

This item will be reviewed by the inspector on a future inspection IFI 518/81-17-01.

The inspector also observed magnetic particle (MT) examination of butt welds on beam number 6202F?. The applicable procedure was QCI N-202 Revision 4.

The AC yoke method was utilizea with 6" pole spacing, calibration was verified and examination was performed by a Level II MT inspector.

Within the areas inspected no violations or deviations were identified.

10.

Status of Inspection and Enforcement Bulletins No new bulletin was received in September, and no information was trans-mitted or action taken which would justify closure of any open, previously received Bulletin.

11. Status of IE Circulars (Closed) 81-08 Foundation Materials Foundation treatments for safety critical structure have been completed.

(Closed for Hartsville) 81-13 Torque Switch Electrical Bypass Circuit for Cafeguard Service Valve Motors. The inspector has assured that site supervisors of project engineering and quality control have been issued copies of this circular, which requires no written response unless discrepancies are found.
