5.

Independent Inspection The inspector examined the following areas:

a.

Soils and concrete laboratory b.

Ongoing work in Units A-1 and A-2 powerblock c.

Nonccaformance Report NCR-HNPA 141 Examination of NCR-HNPA 141 disclosed the following violation: Attachment C to procedure QAP 15.1 in the TVA Division of Construction Program QA Manual requires TVA NCR's to be written wihtin approximately three working days of identification of a defect or deficiency.

Investigations by the NRC inspector disclosed the following examples of failure to follow procedure QAP 15.1:

a.

Hartsville site procedure CEP 15.03, " Control of Nonconformance" does not address the timeliness of identification requirement stated in QAP 15.1 of the TVA Division of Construction QA Manual.

b.

The omission of anchor rods and stiffner plates from the A-1 reactor

drywell frame 10 was discovered in December 1980 and was not identified as an NCR on nonconformance report, NCR-HNPA 141 until May 5,1981.

c.

Failure to address the timeliness requirement for reporting NCR's in procedure CEP 15.03 and to identify the omission of stiffner plates and anchor rods from the A-1 reactor drywell frame 10 as an NCR in the time frame specified by procedure QAP 15.01 was identified to the licensee as Violation number 50-518/81-08-01, Nonconformance Control.

Lakes Dams and Canals - Review of Quality Records, Units A-1, A-2, B-1 and B-2 l

The inspector examined quality records for the central and support fill placed around the A-1 and B-2 spray ponds. Acceptance criteria examined by

Records examined included daily inspection reports, compaction tests and atteberg limit data for Mas E to 11, 1981, and TVA report on the spary pond central and support 'P.'.com October 1 to November 30, 1980.

No violations or deviations were identified.
