Conlon, Section Chief, RC&ES Branch Date signed SUMMARY Inspection on June 12-13, 1979 Areas Inspected This routine unannounced inspection involved 12 inspector-hours onsite in the areas of NSW dam construction work performance and quality records, licensee actions on previous inspection findings, and construction status.
Results:
Of the three areas inspected, no apparent items of noncompliance or deviation were identified in two areas; one apparent item of noncompliance was found in one area (Infraction - Placement of NSW dam filter materials of density less than specified paragraph 3)

Exit Interview The inspection scope and findings were summarized on June 13, 1979 with those persons indicated in Paragraph I above. The licensee acknowledged the noncompliance discussed in paragraph 3.

Licensee Action on Previous Inspection Findings (Closed) Unresolved Item 491/79-09-02, 492/79-08-2 and 493/79-08-02: Relative Density Testing - NSW Dam. Cherokee Specification No. CKS-1114.00-00-0001 requires that filter materials be compacted to a minimum of 80 percent rela-tive density. The licensee has reviewed the test method (ASTM D 1556) used to verify that the filter materials are compacted to at least 80 percent relative density. This review disclosed that an error was made in performance of the quality control test which resulted in compaction of filter materials to less than the minimum specified density. This item is closed as Unresolved Item 491/492/493/79-09-02 and upgraded to Infraction Number 491/79-10-01, 492/79-10-01, and 493/79-10-01, " Placement of NSW Dam. Filter Materials at Densities Less than Specified."

I.akes Dans and Canals - Review of Quality Records, Units 1, 2, and 3 The inspector examined quality records for the fine, coarse and transition filter materials placed in the NSW dam. Acceptance criteria examined by the inspector include PSAR Section 2.5.5 and Appendix 2A, SER Section 2.5.5, SER Supplement No. 1 Section 2.5.3, Specification CKS 1114.00-00-0001, Procedure M-1, M-26, and CKB-21 and Drawing Numbers CK-0026-01 to CK-0026-06.
Records examined were:
Corrected results of field density tests (Sand Cone Method) performed a.
on coare, fine, and transition core materials which have been placed to date in the NSW dam.
b.
Logs of borings drilled at Stations 15 + 12, 15 + 22, and 15 + 72 in the fine filter portion of the chimney drain in the NSW dam.
As a result of questions raised by the inspector in Unresolved Item 491/492/493/79-09-02, the licensee made a review of laboratory and field test procedures to verify the accuracy of sand cone test results (ASTM D 1556). This review disclosed that an incorrect value was used for the bulk density of the standard sand used in the sand cone test.
The sand cone field density test results were recalculated using the correct value for the bulk density. Review of the revised density test results and discussions with responsible engineers disclosed that the revised QC test results indicate that approximately 30 percent of the fine filter material and 50 percent of the transition filter material were compacted to less than the required 80 percent relative density.
As discussed in paragraph 3, this was identified to the licensee as a noncompliance.
Due to the problem discussed above the licensee has under taken an investigation to determine the insita density of the filter materials which have been placed to date. This investigation includes additional in place density tests and drilling of borings and performance of standard penetration tests.
On June 9,1979, the license stopped fill placement in all areas of the NSW dam which contain filter materials. Other area of the dam are not affected since acceptance criteria for other types of fill is based on the drive cylinder method (ASTM D 2937). The licensee will notify NRC Region II prior to resuming work in areas where the filter materials are in place.
