Inspection Summary:

Inspection of December 1980 (Report No. STN 50-482/80-23)

Areas Inspected: Routine, announced inspection by the Resident Reactor Inspector (RRI) including follow uo 'f the corrective action performed for the piping cleanliness work hold agreement and immediate Action Letter; containment steel structures observation of activitie; and records review.

The inspection involved fifty inspector-hours by one NRC inspector.

Daniel International (DI) Organization Chance i

The RRI was informed that the previously proposed DI organizational change has been implemented. The 01 proposal for the new organization arrangement was first presented to the RIV office on June 26, 1980, in Arlington, Texas.

A new position has been created on site, Project

Containment Steel Structures a.

Tendon Sheath Protection i

i The RR! observed that tendon sheaths protruding into the construction opening of the Reactor Building containment wall were not being uniformily protected, in that, approximately one half of the sheaths

had no end covering on them while all others were covered.

In discussions with licensee representatives, it was learned that instal-lation is to be completed utilizing Specification No.10466-C154-A, a nonsafety-related specification. However, paragraph 8.1 requires

that " Splash caps on sheathing covers shall be used to cover all sheathing during the concrete placing operations to prevent concrete and moisture from being splashed into the sheaths." Sharing the RRI'.s concern, the licensee representatives reouested that the contractor place avers on all unprotected sheaths and that thef further, after coupling the sheathing spanning the opening, prior

to pouring concrete, will run a rabbit through all sheathing crossing (vertical and horizontal) the opening.

The RRI indicated that he had no further questions regarding the tendon sheathing in the construction opening, b.

Containment Liner Piate The Chicago Bridge & Iron Company (CB&I) completed the installation of the liner plate in the construction opening of the Reactor Building containment wall on December 12, 1980.

The RRI reviewed the QA records developed by CB&I. The RRI deter-mined, as a result of the record review, that the customer acceptance and release was signed on December 12, 1980, and that copies of the CB&I records were provided to Danie! prior to CB&I's departure (official turnover of records will b e done later by the CB&I corpo-rate office).

Piping Cleanliness Work Hold Agreement As a result of the licensee's Work Hold Agreement No.13 signed and

,

issued on November 21, 1980, the Nuclear Regulatory Commission issued i

Inunediate Action Letters (IAL) on November 26 and December 22, 1980.

!

-1 This work hold agreement stopped final fit-up, welding or bolting of safety-related piping (except reactor coolant loop piping and small

bore piping fabrication) because of unacceptable internal pipe cleanli-ness. The IALs requested that the licensee inform the NRC when corrective action had been completed sufficiently in advance of any plans to resume work so that an evaluation of the new program and its implementation ceald be performed prior to release of the work hold agreement.

The RRI completed this evaluation on December 29-30, 1980, with the

!

following results:

a.

Procedure Modifications (1) Work Procedure WP-VII-201, " Installation of Piping," as

modified by Revision 8 provides:

(a) added responsibilities for the craft foremen to assure cleanliness throughout installation

(b) the specific type of sealing tape to be used for sealing

,

(2) An Interim Change to Procedure ICP-261, Revision 0 has been issued for use with Quality Control Procedure QCP-VII-201,

Revision 3 " Installation and Fabrication of Piping, Components and Supports," regarding the Quality Control inspection coverage and sign-off requirements modifications as well as new surveil-lance of c'eanliness control activities by others.

ICP-262. Revision 0 issued for use with Work Procedure WP-VII-203, Revision 3, " Cleaning and Sealino Pipe," which modifies previous instructions regarding cleaning practices and activities.

Issuance of a construction memo No. CM 424 on December 22, 1980, providing detailed instructions to all affected craft personnel of their new responsibilities regarding cleanliness and their

,

need to review,the procedures involved.

The RRI further learned that craft foremen read the memo to their craft personnel during a safety meeting and further that all craft personnel were

'

required to sign that they had read the memo.

The RRI conducted a survey of all the procedure review stations

,

located thrcughout the site and determined that more than one copy of all of the above listed procedure revisions were available at all of the stations and that the station daily review logs reflected that review was being completed by craft personnel.

Training Activities (1) A special video training film was prepared for use in the training sessions that covered all the new cleanliness requirements and responsibilities. A special showing of this training film was attended by the RRI.

(2) Training was conducted for all involved personnel which included using this film.

In addition, other special training was con-ducted for craft supervisors regarding their responsibilities.

Testing was performed afterwards which all craft supervision must pass.

The RRI was informed that all supervisors had successfully passed the test.

The RRI determined that all training had been completed and that controls were in place for proper training of new hires and trans-ferred personnel as required.

c.

Provisions for Additional Monitoring and Surveillance as Well as Other Instruction Aids (1) A Construction Monitor (Mechanical) has been appointed, properly trained and assigned to the work area.

His responsibilities are

outlined in Appendix I of WP-VII-201, Revision C.

(2) Surveillance of work activities instructions are in place for Quality Control inspectors in addition to Quality Assurance auditors to perform cleanliness activities surveillance.

(3) Hard cards have been prepared and issued, to all involved personnel, of a small size, handy for craft personnel to

The RRI determined that the above items have been completed.

Based on the above determination by the RRI, as documented in IAL of

i December 31, 1980, the licensee removed the work hold agreement at

14:00 hours on December 31, 1980.

Exit Meetings J

l The RRI attended an investigation exit meeting' conducted by a RIV investigator at 8:15 on December 12, 1980.

'The RRI met with licensee representatives (denoted in paragraph 1) on December 18, 1980. The scope and findings of the December inspections
