Inspection on March 31 - April 3, 1980 (Report No. STN 50-482/80-07)
Areas Inspected: Routine, unannounced inspection of licensee action ou pre-vious inspection find # :gs; construction deficiency reperts; an allegation in regard to welding; and current construction activity. The inspection involved twenty-three inspector-hours on site by one NRC inspector.
Results: No items of noncompliance or deviations were identified.

Licensee Action on Previous Inspection Findings (Closed) Unresolved Item (STN 50-482/78-13):
Consistency of Admixture Material Properties.
The IE inspector reviewed the infrared spectrum analysis conducted by Masters Builders, and supplied to Daniel on admixture shipments for May 8 and October 4, 1978; July 13, November 5, 14 and 23, 1979.
Material properties were shown to be consistent with this time frame.

or Unapproved Procedures.
The IE inspector verified the issuance of KG&E Surveillance Report S-71.
Further review by KG&E QA staff has not revealed the existence of similar type uncontrolled procedures.

(Closed) Noncompliance (STN 50-482/79-01): Failure to Sample the Concrete Batch Subsequent to Rejectable Batch.
Daniel Procedures QCP-IV-106 and WP-IV-106 contain appropriate procedures for control of concrete deliveries when air and/or slump tests are not passed.
The KG&E Site QA group has conducted a review of the 1979 placements in order to determine if the revised program is effective and te confirm that " follow loads" are tested. The results of their review, which are documented in KG&E Surveillance Reports S-99 (July 10, 1979) and S-200 (April 2, 1980)
(draft), indicate that reject concrete is be.'ng identified and prevented from being used in safety-related placements.

(Closed) Noncompliance (STN 50-482/79-04): Tempersture Control in Concrete Lab Curing Room.
The IE inspector verified that improvements in control of the concrete curing room have been implemented. Additional equipment to control temperatures has been obtained and personnel trained in monitoring operations of curing room temperature controls and recording devices.

(Closed) Construction Deficiency Report (STN 50-482/79-01):
Safety-Related Valves. All four Anchor-Darling valves delivered to Wolf Creek have had their valve actuators replaced.
This completes Wolf Creek action on the Part 21 report issued by Achor Darling Valve Company on December 6, 1978.

Specification Require-ment for Testing of Dry Pack Mortar.
Bechtel has implemented appropriate requirements for testing of dry pack mortar in specification No. 10466-C191.

(0 pen) Unresolved Item (STN 50-482/79-20): Fuel Building Concrete.
On November 23, 1979, the IE inspector, in company with the KG&E Site QA manager,. identified cracked and broken concrete in the fill slab for the fuel transfer canal.
This was subsequently documented in a deficiency report, ISD2863C of November 29, 1979.
Recent inspection of the areas involved were made by KG&E personnel and additional cracking was found and documented in NCR ISN1874C of March 26, 1980.
Bechtel is taking action to provide a solution to the nonconformance.
The item will remain unresolve'd until action by the A/E has been completed and reviewed.

Moore Personnel. A revision (Rev. 1, dated April 7,1980) to the Project Quality Assurance Plan (Dames & Moore - Wolf Creek) requires specific qualification verification of Dames & Moore personnel to KG&E.

(Closed) Unresolved Item (STN 50-482/79-07):
Polar Crane Support Installation.
An NCR was issued on May 1, 1979, regarding the welding of the corner cope in polar crane supports.
The A/E determined that the supports may be used as is in that the design intent has been satisfied.

Site Tour, The IE inspector toured the construction areas to observe general construction activities and to inspect housekeeping.
No items of noncompliance or deviations were identified.
4.
Spalled Concrete in Reactor Cavity - Potential Construction Deficiency Report (10 CFR 50.5S(e))
Spalled and broken concrete has been identified in the ledge around the reactor cavity.
It was first identified on February 27, 1980, and thought to be caused by the swelling of tne 2" x 4" lumber which was used as a form for a drain collector trench.
Subsequent examination and chipping of the area as specified in DR ISD3411C (March 3, 1980) revealed an area which exceeded that allowed to be repaired by the construction force under Specification C-103.
A Nonconfarmance heport (N"R) was issued on March 10, 1980.
On March 24, 1980, Bechtel requested additional information to be supplied on the deficiency in order to completely disposition the NCR.
This item is considered unresolved and will be further reviewed during a subsequent inspection after receipt of the NCR resolution by Bechtel.
5.
Allegation of Defective Welding On March 3, 1980, Region IV received a telephone call regarding defective welding performed on a "rubstructure ring." The involved welder's name was supplied. Upon investigation of this allegation, the following was determined:
The item involved was a seal ring in the refueling canal, a.
(stainless steel plate, inch) where it attaches to the reactor cavity liner (carbon steel plate, inch).

The event was reported to the NRC on January 4, 1980.
NCR No. INN 1551M was issued by the site on December 21, 1979.

A small quantity of E308 electrodes were used instead of E309L.

This transition weld is not safety-related in that it is not a pressure boundry The IE inspector concluded that the item is not safety-related and therefore outside the jurisdiction of the NRC. No items of non-compliance or deficiencies were identified.
6.
Unresolved Item Unresolved items are matters about which more information is required in order to ascertain whether they are acceptable items, items of noncompliance, or deviations. Unresolved items disclosed during the inspection are discussed in paragraphs 2 and 4.
7.
Exit Interview The IE inspector met with the licensez representatives (denoted in paragraph 1) at the conclusion of the inspection on April 3,1980.
The inspector summarized the scope and findings of the inspection.
