Inspection during September 30 - October 1, 1980 (Report No. 50-482/80-19)
Areas Inspected:
Routine, unannounced inspection of the planned safety-related concrete placement of the Reactor Containment Building Dome.
The inspection involved eight inspector-hours on site by one NRC inspector.
Results: No items of noncompliance or deviations were identified.

Taylor, Concrete Technical Superintendent The IE inspector also interviewed other licensee employees and contractor personnel including members of the QA/QC and engineering staffs.

Concrete Construction The IE inspector performed a review of the Daniel International Corporation "Special Instruction for Placement and Consolidation of Concrete" for Placement No. OC281-501, the Reactor Containment Building (RCB) dome slab, elevation 2185'-11b" to 2208'-0".
In addition, Daniel Drawing No. FD-C-556, Revision 0, " Concrete Placing Plan - Reactor Building Final Dome Pour," was also reviewed. The placement area was inspected while final cleanup operations were in progress. The IE inspector also attended the final preplacement meeting held the day prior to the scheduled placement. During this meeting, a Nonconformance Report (NCR) was identified which caused the scheduled placement to be postponed indefinitely until resolution could be obtained. The NCR was generated because the as-built documentation did not include field welded items attached to the RCB liner ceiffeners, such as. tendon sheathing supports. Bechtel Drawing No. C-OL2311, Revision 8, " Liner Plate," requires that all field welding of items perma-nently attached to stiffeners be subject to permanent records. The Resident Reactor Inspector (RRI) was informed of the delay in placement and its cause by the IE inspector. The RRI performed a follow-up inspection during the actual placement of the concrete.
No items of noncompliance or deviations were identified during this inspection.
3.
Exit Interview The IE inspector met with the licensee representative denoted in paragraph 1 at the conclusion of the inspection on Octcber 1,1980.
