On September 30, 2023, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Clinton Power Station. On October 23, 2023, the NRC inspectors discussed the results of this inspection with Norha Plumey, Site Vice President, and other members of your staff. The results of this inspection are documented in the enclosed report.

No findings or violations of more than minor significance were identified during this inspection.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Clinton Power Station, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. Refer to https://www.nrc.gov/reactors/operating/oversight.html for more information.

PLANT STATUS

Unit 1 began the inspection period in Mode 1, operating at or near full-rated thermal power. On August 15, 2023, the unit began end-of-cycle coastdown to the planned refueling outage (C1R21). On September 11, 2023, the unit reduced power to approximately 15 percent full-rated thermal power and inserted a manual reactor scram to enter Mode 3 (hot shutdown) to commence C1R21. Following an orderly shutdown, depressurization, and cooldown, the unit was placed in Mode 4 (cold shutdown). The unit remained in cold shutdown for the remainder of the inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed activities described in IMC 2515, Appendix D, Plant Status, observed risk significant activities, and completed on-site portions of IPs. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

71152S - Semiannual Trend Problem Identification and Resolution Semiannual Trend Review (Section 03.02)

(1) The inspectors reviewed the licensees corrective action program for potential adverse trends in foreign material exclusion practices that might be indicative of a more significant safety issue. For this sample, the inspectors review focused on licensee corrective action reports initiated from the February 2023 forced outage (C1F63) through the September 2023 refueling outage (C1R21).

INSPECTION RESULTS

Observation: Suppression Pool Foreign Material Exclusion Semiannual Trend Review 71152S The inspectors performed a review of plant issues, particularly those entered into the licensees corrective action program (CAP) over several months following the forced outage, C1F63, which ended in February 2023. During their review, the inspectors noted a potential adverse trend involving foreign material exclusion (FME) behaviors around the suppression pool. Specific CAP entries reviewed by the inspectors included, but were not limited to:

  • AR 4554341, FME EOID Object Floating in Supp Pool Near Personnel Airlock, dated February 13, 2023
  • AR 4560498, FME: Ink Pen in Suppression Pool, dated March 8, 2023
  • AR 4681285, FME - Object Floating in Suppression Pool, dated May 29, 2023 The licensee identified a related trend on FME behaviors, documenting the behaviors listed above among other identified FME behaviors in their CAP as AR 4686214, Trend IR:

Behaviors in FME, on June 22, 2023. Following the initiation of that AR, there were a number of additional ARs written against FME behaviors seen below:

  • AR 4688117, FME: Ear Plug in Suppression Pool, dated July 1, 2023
  • AR 4689208, FME - Something Floating in Suppression Pool, dated July 7, 2023
  • AR 4690769, FME: EOID Debris Found Floating in Suppression Pool, dated July 15, 2023
  • AR 4692457, FME - Objects Floating in the Suppression Pool, dated July 25, 2023
  • AR 4697167, FME - Object Floating in Suppression Pool, dated August 17, 2023
  • AR 4704574, C1R21 FME - Objects Floating in the Suppression Pool, dated August 23, 2023 On August 24, 2023, the licensee determined that a trend still exists, and a new trend AR was written to correctly address behaviors. The new AR trend was documented in CAP as AR 4699456, Trend IR: FME Behaviors, on August 29, 2023. The corrective actions include performing a work group evaluation (WGE) to evaluate if additional training is required for FME, ensuring contractors are being briefed about FME controls, and evaluating if additional barriers are required prior to entering the FME zone. The WGE was in progress at the close of the inspection period and was not reviewed by the inspectors.

After reviewing the reports, the inspectors found that there was an apparent increasing trend in FME events with regards to the suppression pool. However, the licensee properly identified the need for trend review and have been generating corrective actions consistent with MA-AA-716-0008, "Foreign Material Exclusion Program," procedure. The inspectors did not identify a performance deficiency and have determined the licensee is in compliance with their corrective action process. No findings or violations of NRC requirements of more than minor safety significance were identified in the course of this review and an existence of a more safety significant concern was not identified.
