Licensee Action on Previously Identified Items (Closed) 10 CFR 50.55(e) Report No. 50-461/77-02. Subject:

Breakdown in Quality Assurance concerning procurement of service water piping.

The inspector reviewed the " Investigation of Reportable Deficiency,"

Y-2016, L14-PY-77 (08-29) -9 explaining the cause of the deficiency and action taken to prevent further breakdown in procurement activ-ities.

In addition the licensee has used concrete bash fill at the buried portion of the service water line and installed a cathodic protection system to control corrosion.

(0 pen) 10 CFR 50.55(e) Report No. 50-461/81-01. Subject: Lack of control of preheat of main steam line closure spool welding to the reactor safe ends.

The inspector reviewed the following:

a.

Letter GE to L. Dozier, GSI-013-81, dated 1/7/81, stating problem and lift of stop work order and that a detailed investigation is underway and the results will be forwarded to you when received.

b.

GE FDDR No. LH-163-81 dated 1/12/81, stating that surface hardness readings indicate that hardness was unaffected.

c.

Letter IPC to GE, DEK-836-IPC, dated 1/12/81, stating observa-tions made.

This item remains open pending the licensee obtaining a copy of the engineering analysis performed by GE (see item a. above)

for review by NRC Region III.

(Closed) 10 CFR 50.55(e) Report No. 50-461/80-03, Subject:

Break-down in QA concerning procurement of Rosemount pressure transmitters in that the transmitters did not meet environmental qualification per IEhE-324-1974.

The inspector reviewed the following:

a.

Record of coordination dated March 19, 1980, stating that GE's PQC's for the transmitters did not indicate any noncon-formances while the transmitters were not environmentally qualified.

Eight GE PQC's Revision 1, revised to reflect that the transmitters were not environmentally qualisted, c.

Baldwin Associates FCR No. 4768 dated 6/9/81.

This item appears to be in conformance with IPC's final report to NRC, Region III, U-0137, L14-80(04-30)

-9, and is considered closed. Other deficiencies with Rosemount pressure transmitters are the subject of IE Bulletin No. 80-16 and IE Circular 80-16 which remain open.

(Closed) Noncompliance (50-461/80-02-04). The inspector reviewed the following:

Baldwin Associates QA No. 140 Revision 4, dated 10/6/81, which now requires a QA NCR review.

Baldwin Associates QAM Section 13, "Nonconformances" Revision 2, dated 4/19/82, paragraph 13.3.2 C which provides for generic corrective action.

This item is considered closed as the licensee appears to have adequate control and overview of nonconformances, corrective actions and trend analysis systems.

(Closed) Licensee tracking No. 50-461/81-23. This item concerns a management meeting report wherein the licensee committed to take certain actions (see report No. 50-461/81-03, item No. 2).

The inspector considers this item closed as the licensee appears to have honored his commitments.

IE Bulletins (Closed) 50-461/78-04 - Environmental Qualification of Certain Stem Mounted Limit Switches Inside Reactor Containment. The inspector reviewed IPC's letter to NRC, Region III, U-0069, LO8-PU-78(04-18),

stating that NAMCO D 2400X and EA-170-320 Snap Lock Switches will not be used in any safety related equipment in primary containment at CPS.

(Closed) 50-461/78-12-12A-12B - A Typical Weld Material in Reactor Pressure Vessel Welds.

The inspector reviewed IPC's letter to NRC, Region III, U-0095, LO8-79 (05-29)-9 and CB & I's generic report to I&E.

(Closed) 50-461/80-02 - Inadequate Quality Assurance for Nuclear Supplied Equipment. The inspector reviewed IPC's response letter to NRC, Region III, LO8-80(02-01)-0, stating that CPS uses feedwater spargers fabricated by CB&I that were purchased in May 1977. Action items 2 and 3 were not applicable as Clinton Unit No. 1 is not operational.

(Closed) 50-461/80-04 - Analysis of a PWR Main Steam Line Break With Continued Feedwater Addition. This bulletin is not applicable as Clinton Unit No. 1 is a BWR.

(0 pen) 50-461/80-17 thru Supplement No. 5 - Failure of 76 of 185 Control Rods to Fully Insert During a Scram at a BWR. The inspector reviewed a letter from IPC to Standardization and Special Protects Branch, Division of Licensing, USNRC, U-0342, L 17-81(12-03)-6, with attached "LRG-II Position Papers Volume II" dated 12/3/81.

This bulletin remains open pending closcout by Standardization and Special Projects Branch.

(Closed) 50-461/81-02-with Supplement No. 1 - Failure of Gate Type Valves to Close Against Differential Pressure. The inspector re-viewed IPC letter to NRC, Region III, U-0293, LO8-81(09-23)-6, stating that CPS has no safety related motor operated gate valves manufactured by Westinghouse - EMD.

(Closed) 50-461/81-03 - Flow Blockage of Cooling Water to Safety System Components by Corbicula Sp. (Asiatic Clam) and Myfilus Sp.

(Mussel). The inspector reviewed IPC letter to NRC, Region III, U-0256, LO8-81(07-14)-6, stating that IP has a proposed system for controlling clams should they become a problem.

IE Circulars (Closed) 50-461/77-10 - Vacuum Conditions Resulting in Damage to Liquid Process Tanks.

IP's circular status sheet states that Rad waste tcnks (35) are all vented. Other possible detrimental effects mitigated since the Area Radiation Monitors / Process Radiation Monitors monitor for release of activity, and berms / curbs direct liquid releases to drains for processing.

(Closed) 50-461/77-12 - Dropped Fuel Assemblies at BWR Facilities -

IPC's circular status sheet states that, " low load sensing ongrapple cable; speed reducer is coupled to hoist drum which has light to indicate positive engagement."

(Closed) 50-461/78-06 - Potential Common Mode Flooding of ECCS equipment rooms at BWR Facilities - IPC's circular status sheet states that, "each ECCS room h9s its own sumps and pumps that pump through check valves to rad waste.

There is no common header as described in the circular."

(Closed) 50-461/78-15 - Tilting Disc Check Valves Fails to Close with Gravity in Vertical Position.

The inspector reviewed S&L letter to IPC, SLMI-5013, dated 7/30/80, stating that it was de-termined that only 3 check valves were installed in the vertical risers only one was of the type discussed in the circular. Anchor Darling advised that this one valve does not have the tilting disc design; is designed such that the center of gravity does not allow the valve to hang open.

IPC's cire.ular status sheet states that, "no GE switchgear shipped to site between 8/1/78 thru 7/1/79. All other switchgear furnished by Westinghouse."

(Closed) 50-461/79-21 - Prevention of Unplanned Releases of Radio-activity - IPC's circular status sheet states that, "IPC is in com-pliance with the circular."

(Closed) 50-461/80-10 - Failure to Maintain Environmental Qualifica-tion of Equipment - IPC's circular status sheet states that, "IPC is to comply with NUREG 0588."

(0 pen) 50-461/80-16 - Operational Deficiencies on Rosemount Model 510DU Trip Units and Model 1152 Pressure Transmitters. This circular remains open pending licensee verification of the modification.

(Closed) 50-461/81-02 - Performance of NRC Licensed Individuals While on Duty - The inspector reviewed IPC's " Document Review Form" dated 2/23/81 indicating that prospective licensed operational personnel had reviewed the circular.

(Closed) 50-461/81-05 - Self Aligning Rod End Bushings for Pipe Supports - IPC's circular status sheet states that, "all bearings have been staked at the factory, Pacific Scientific; as preemptive corrective action, also preservice inspection program will include visual inspection of snubbers."

(Open) 50-461/81-06 - Potential Deficiency Affecting Certain Foxboro 10 to 50 Mil 11 ampere Transmitters - This circular is open pending final action by the licensee.

(Closed) 50-461/81-09 - Containment Effluent Water that Bypasses Radioactivity Monitor - IPC's circular status sheet states that,

" CPS cooler fan motors are air cooled. No other water system effluent problems were noted in the review."

(Closed) 50-461/81-12 - Inadequate Periodic Test Procedure of PWR Protection System - This circular is not applicable to CPS.
