Introduction Mr. Keppler opened the meeting by discussing concerns being expressed by Congress and the Commission relative to the need for utility management to be more heavily involved in the Quality Assurance aspects of their plant.
Mr. Eisenhut discussed the additional requirements being placed on NTOL plants including QA commitments, additional staffing and reviews by a third party organization.

NRR QA Concerns The Quality Assurance Branch, NRR, summarized their concerns relative to the Illinois Power Company (IPC) response to H. Denton dated September 30, 1981.
These concerns involve the overall QA management approach being used by IPC as demonstrated by:
1.
QA responsibilities being fragmented throughout many IPC organizational departments.
2.
Lack of QA involvement in evaluating and determining accept-ability of inspection and test results.
3.
Lack of QA involvement in such areas as:
a.
Inservice Inspection b.
Noncompliance and Corrective Actions c.
Identification and Control of Items d.
Special Processes I

Lack of QA involvement in reviewing QA/QC procedures and controls in other IP and BA departments.
It was recommended by the QAB that IPC should reassess their QA philosophy towards the Clinton project, including the QA Program for Operations; to have IPC senior management review other sites with problems similar to Clinton; and, to bring in a consultant in QA to help IPC reshape the QA organization and the QA program.

Summary of Region TIT Staffing Concerns Mr. Holzmer noted that the staffing levels for the on-site organization were about 20% or more below those of other single unit NTOL applicants overall. Particular areas of concern were plant training, QA/QC, and technical departments. Comparison was made to single unit facilities in Region III who had increased their staffing, partially as a consequence of their regulatory history. The staff stated that the applicant would be required
to provide justification for their projected staffing levels.

Relationship of Construction QA Concerns to Operational QA Concerns Region III personnel expressed the concern that many of the QA problems currently existing at the Clinton site in the construc-tion area are due to the management philosophy of Illinois Power Company and, as such, the same type of QA problems could be

generated in the operational QA organization.

Some root causes of the current problems involving cable trays and hangers were perceived by the NRC to be attributable to the following:
1.
A lack of IPQA involvement.
Lack of IPQA not rev! ewing other department procedures such as Baldwin Associates QCI procedures to assure that quality aspects of these procedures are in conformance with IPQA philosophy.
A lack of knowledge of the current electrical QA problems coupled with the closcout of a similar construction deficiency report at the same time indicates inadequate QA involvement in what is happening in the plant.
2.
A general weakness in identifying root causes. An example was the magnitude of the current electrical cable tray problems, to a large extent, has been caused by the lack of trending due to the use of the informal G.I.R. system to document nonconforming conditions.
3.
A lack of prompt corrective action.
It was pointed out that the failure to control a large number of small noncon-forming conditions led to the current electrical problems and that had prompt corrective action been taken on these items as they were found, the current work stoppage would not have been necessary.
4.
Attitude towards QA/QC. Based on conversations with senior IPC management, there was a perception by the NRC that QA/QC was not to hold up the job and that quality findings were handled to minimize schedule considerations rather than quality considerations.
5.
Intimidation and Alienation of QA/QC Personnel.
IPC was informed that there was a general perception among QC inspectors that they were taking a chance on losing their jobs if they cooperated with the NRC. This, plus the recent firing of two QC inspectors has continued an aliena-tion which has been observed at the site since the team inspection conducted in February 1981.
E.
Recent Inspection Findings A preliminary review of the apparent items of noncompliance found during the current investigation was presented. These items will be detailed in Investigation Report No. 50-461/82-02(EIS).
F.
Illinois Power Company Response The licensee stated that they understood the NRC concerns and th'at it was their intent to address these concerns.
It was agreed that periodic meetings would be held to keep everyone informed of the QA program changes.
