Results: Within the areas inspected, one violation was identified (failure to identify an inoperable fire door and establish the required fire watch, paragraph 4).

Licensee Event Report (LER) Followup Through direct observation, discussions with licensee personnel, and review of records, the following fire protection / prevention related LERs were reviewed to determine that reportability requirements were fulfilled, immediate corrective action was accomplished, and corrective action to prevent recurrence had been accomplished.
LER 86-012 reported the inadvertent cancellation of fire watch patrol in two remote shutdown panel rooms.
The fire watch was reestablished when the error was identified.
The licensees permanent corrective action was to require the fire watch supervisor to review firewatch route changes with the firewatch foreman and to further require that the cognizant maintenance supervisor review all fire watch positions and patrol routes to ensure that adequate coverage is maintained.
This LER is closed.
LER 86-031 reported the cancellation of a fire watch patrol upon completion of requirements for an LCO when another LCO, still incomplete, required the same fire watch.

The licensees permanent corrective action was to revise procedure FPP-070 " Fire Watch Duties" to ensure that hourly fire watches covered by more than one LC0 were clearly denoted as such.
This LER is closed.
LER 86-036 reported the discovery of improperly located internal conduit fire seals rendering the fire barrier penetration seals inoperable. The licensee immediately established plant wide fire watch patrols until the scope of the problem could be defined.
Permanent corrective action was to complete an acceptable seal for each identified problem penetration. This LER is closed.
LER 86-038 reported that an unsealed penetration was found by QA and subsequently a total of 56 penetrations were found not sealed. This error Wds traced to the deletion of these seals from the design documents because they had been identified as not requiring radiation seals.
Fire Wdtch patrols were already in effect as a result of the problem identified in LER-86-036. The penetrations were sealed and reidentified on the design documents. This LER is closed.
LER 85-013 reported on the discovery that a DC power supply for the remote shutdown panel was found located in the main control room. This was not in accordance with the licensee's commitments to 10 CFR 50, Appendix R.
Corrective action was to relocate this power source to a panel outside of the control room prior to attaining 5 percent power. The NRC inspector reviewed the completed MR Package 85-0655 dated October 13, 1985. The work was completed satisfactorily.

This review verified that the licensee had technically adequate procedures to implement the fire protection program.
Procedural guidance was provided to control combustible material and reduce fire hazards.
Administrative procedures also provided for maintenance and surveillances on fire suppression, detection, and support equipment.
Personnel training, qualifications, and responsibilities were adequately provided for. Maintenance evolutions that significantly increase fire risk were properly controlled.

The NRC inspector toured accessible areas of the plant site for general area condition, work activities in progress, and visual condition of fire protection systems and equipment.
Combustible materials, flammable and combustible liquid, and gas usage were restricted or properly controlled in areas containing safety-related equipment and components.
Items checked included positions of selected valves, fire barrier condition, hose stations, hose houses, halon systems lineups, fire lockers and fire

extinguishers for type, location, and condition.

During this tour, the NRC inspector noted a number of fire doors with varying degrees of modifications.
This condition was discussed with i
licensee representatives in that the fire doors were rated and labeled as y
units by the specifications; each unit being comprised of the door, frame i
and hardware as delivered from the vendor.
Any changes, additions, or other modifications to the units can compromise the unit label rating.
This matter will be considered an unresolved item pending review and
action by the licensee to identify acceptable and unacceptable fire door conditions and review by the NRC during a subsequent inspection.

-5-One fire door, CB-116-23, was found in an inoperable condition with a missing lock mechanism and disabled closure hardware.
Further investigation revealed that this fire barrier penetration was not listed as requiring a fire watch.
Failure to satisfy technical specification requirements for an inoperable fire barrier penetration is an apparent violation.

There was no welding, cutting, or use of open flame ignition sources found in the areas toured. There were no construction activities in progress in the toured areas; however, there were some maintenance work and surveillance testing in progress. General housekeeping conditions were found to be satisfactory.
Fire protection systems and equipment installed for protection of safety-related areas were found to be functional and tested in accordance with the requirements specified in the technical specifications.
Fire brigade equipment, including emergency breathing apparatus, was found to be properly stored and maintained.
The NRC inspector also reviewed fire brigade training and drill records and reviewed the current roster of qualified brigade members. The records were in order and confirmed that training and drills were being conducted at the specified intervals.
4.
Unresolved Items An unresolved item is a matter about which more information is required in order to determine whether it is acceptable, a violation, or a deviation. One unresolved item is discussed in this report in paragraph 3.
5.
Exit Interview An exit interview was conducted on September 26, 1986, with those personnel denoted in paragraph 1 of this report. At this exit interview, the NRC inspector summarized the scope and findings of the inspection.
