Licensee Action on Previous Inspection Findings a.

Improper disposition of nonconformance reports (NCRs).

The NRC inspector reviewed the revised NCRs and determined that they were properly dispositioned.

Failure to update procedures after issuance of three Temporary Change Notices (TCNs)

The NRC inspector reviewed system operating procedures and determined that all procedures had less than three TCNs except one which had a revision in progress.

(Closed) Violation (458/8566-02): Failure to properly prepare TCNs.

The NRC inspector reviewed River Bend Procedure ADM-003 to ensure that procedure required incorporation of previously approved TCNs when preparing a new TCN. The inspector sampled recently issued TCNs to verify that ADM-003 was being followed.

3.

Licensed Operator Training The purpose of the inspection was to evaluate the effectiveness of the licensed operator training program. The licensee stated that they are committed to provide their operator training program to the Institute of Nuclear Power Operations (INPO) in November 1986.

The NRC inspector reviewed River Bend Procedure TPP-7-011, " Licensed Operator Requalification Training," Revision 0, dated March 4,1985, to determine the adequacy of the program for licensed operator requalification. Review of requalification lecture schedules, attendance records, periodic tests, and training records of several licensed reactor operators and senior reactor operators determined that required subjects are being taught, all required personnel are attending required lectures, and training records are being adequately maintainsd. Records of control manipulations are being maintained and are regularly updated by the training department. The operations department has made a concerted effort to assure that all operators attend the required requalification lectures and has made provisions to ensure that all personnel who have missed lectures attend scheduled makeup lectures. The NRC inspector reviewed lesson plans and attended requalification lectures to assure that technical content of information presented is adequate. Lesson plans were found to be adequate for material presented. Lectures were given in a thorough, professional manner and followed the lesson plans.

Lesson plans were changed as required to reflect operating experience and any special needs of the operators. The first annual requalification exam is scheduled for November 1986.

The NRC inspector reviewed River Bend Procedure TAP-5-005, " Configuration Management of Training Systems," Revision 1, dated January 2, 1986, to determine the adequacy of the program for ensuring operational problems and lessons learned are included in the requalification program.

The training department reviews all condition reports, Licensee Event Reports (LER's) and modification requests to determine requirements for inclusion in the requalification program.

Interviews with training department and operations department personnel showed that good communications exist between the departments for allowing operations input into the determination of training _needed.

No violations or deviations were identified in this portion of the inspection.

4.

Nonlicensed Staff Training The purpose of this portion of the inspection was to evaluate the effectiveness of the training programs for the nonlicensed staff.

05/09/86 The administrative procedures were reviewed to determine that responsibility was clearly assigned for administering and evaluating related training.

The review also determined that means existed and that any lessons learned from events / activities could be factored into the training program.

The lesson plans were reviewed to verify that specific training would be of sufficient scope to enable personnel to effectively and safely discharge their duties.

Personnel were interviewed to determine that training did adequately prepare them for their jobs in the areas of maintenance, administrative controls, radiological health and safety, security, and quality assurance.

Training records were reviewed for 20 nonlicensed staff members.

The records reviewed verified that individual qualifications were in accordance with regulatory requirements and licensee commitments. The review also verified that required training had been successfully completed.

The licensee has committed to a training program accreditation by INP0.

The-licensee appears to be progressing as scheduled toward that goal. In the area of nonlicensed staff training, no violations or deviations were identified.

The licensees training program and its implementation appear to have been meeting regulatory requirements and licensee.:ommitments.

5.

Exit Interview An exit interview was held on May 16, 1986, with the personnel denoted in paragraph 1 of this report.

The NRC senior resident inspector also attended this meeting.

At this meeting, the scope of use inspection and the findings were summarized.
