BYRON STATION - INTEGRATED INSPECTION REPORT 05000454/2024002 AND 05000455/2024002 AND EXERCISE OF ENFORCEMENT DISCRETION

Dear David Rhoades:

On June 30, 2024, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Byron Station. On July 8, 2024, the NRC inspectors discussed the results of this inspection with H. Welt, Site Vice President, and other members of your staff. The results of this inspection are documented in the enclosed report.

One finding of very low safety significance (Green) is documented in this report. This finding involved a violation of NRC requirements. We are treating this violation as a non-cited violation (NCV) consistent with Section 2.3.2 of the Enforcement Policy.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Byron Station, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. [H.5] - Work Management 71152A A self-revealed finding of very low safety significance (Green) and associated non-cited violation (NCV) of Technical Specification 5.4.1.a was identified for the licensees failure to establish adequate maintenance work instructions to replace the 7R cylinder rocker arm assembly on the 2A Emergency Diesel Generator (EDG). Specifically, maintenance planners failed to include appropriate instructions in safety-related work order (WO) 5282402, 2DG01KA 6 Year Mechanical Inspection, to install 1/4 plugs on the shaft of the rocker arm assembly during a 2A EDG work window. This condition caused a leak of lube oil from the rocker shaft resulting in improper lube oil flow to the rocker arm lifter and subsequent additional unavailability time to necessitate identification and repair of a collapsed lifter.

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed activities described in IMC 2515, Appendix D, Plant Status, observed risk significant activities, and completed on-site portions of IPs. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

INSPECTION RESULTS

Enforcement Discretion Enforcement Action EA-24-053: Failure to Comply with 10 CFR Part 37 71124.08

Description:

On April 27, 2016, the licensee was issued NRC Inspection Report Nos. 05000454/2016406 and 05000455/2016406, which documented a licensee identified violation of 10 CFR Part 37, Physical Protection of Category 1 and Category 2, Quantities of Radioactive Material at Facilities with a 10 CFR Part 73, Physical Protection Program. The violation met the criteria for Enforcement Discretion as described in Enforcement Guidance Memorandum (EGM)14-001, Interim Guidance for Dispositioning 10 CFR Part 37 Violations with Respect to Large Components or Robust Structures Containing Category 1 or Category 2 Quantities of Material at Power Reactor Facilities Licensed Under 10 CFR Parts 50 and 52.

Subsequently, the inspector re-evaluated this activity and found that while a violation of regulatory requirements continues to exist, the conditions remain within the criteria for enforcement discretion established within EGM 14-001.

Corrective Actions: As specified in EGM 14-001, the application of discretion is authorized until the underlying technical issue is dispositioned through rulemaking or other regulatory action.

Enforcement:

Violation: On April 27, 2016, a violation of 10 CFR Part 37 was documented in Byron Inspection Report Nos. 05000454/2016406 and 05000455/2016406. The inspectors determined that the previously identified violation remains.

Description:

At 1200 on April 21, 2024, the station entered limited condition for operation (LCO) 3.8.1, AC Sources - Operating, for a planned 6-year preventative maintenance work window for the 2A EDG under WO 5282402, DG01KA 6 Year Mechanical Inspection. Early into the work window, it was identified that additional maintenance was necessary on the 6R and 7R cylinders of the diesel due to damage sustained to the lifters over the previous operating period. Further inspection found that the entire rocker arm assembly for the 7R cylinder would need to be replaced due to mechanical binding. A major revision was added to the WO to include the additional maintenance.

The rocker arm assembly was replaced part way through the work window and prior to the performance of a series of post-maintenance tests (PMT). During one of the PMT runs on April 29, 2024, operators observed the 7R cylinder temperature to be slightly elevated high out of band. Maintenance technicians removed an inspection cover of the 7R cylinder while the EDG was running and observed lube oil leaking from both ends of the rocker arm shaft, as documented in AR 4770407, Lube Oil Leak on 7R Cylinder from Rocker Arms. The EDG completed the run as the station investigated the cause of the oil leak.

Drawing KSV-25-4, Inlet and Exhaust Rocker & Push Rod, was discovered to show 1/4 plugs installed on either end of the rocker shaft as necessary components of the assembly. These plugs had not been incorporated into the maintenance package during the planning stages after discovery. Purchase and stocking information for the plugs was identified and searches found recent, similar work from Braidwood for a rocker arm assembly where the plugs had been identified and incorporated into the work package. A revision to the WO was made to install the plugs. The EDG successfully completed a PMT run and the LCO was exited at 2131 on April 29, 2024.

The station re-entered the LCO at 2223 on May 1, 2024, to perform additional maintenance.

The LCO was exited at 0708 on May 2, 2024, following a confidence run. During this run, additional maintenance was identified and the LCO was again entered at 0905. During a PMT run following this maintenance, operators observed the 7R cylinder was making more noise than usual and that a noticeable knock could be felt by hand on the access panel. This was captured in AR 4771539, 2A EDG excessive clattering on the 7R cylinder valve train. While not expected nor normal, the licensee concluded that the clattering noise did not represent an unacceptable condition that would impact the EDGs availability or operability, and that the 2A EDG was capable of performing its specified function. The PMT run was completed and the LCO exited at 1106 on May 2, 2024.

A maintenance strategy was developed to diagnose the cause and perform repairs as necessary. The station entered the LCO later in the afternoon of May 2, 2024, at 1329 to troubleshoot and discovered a failed valve lifter. Drilled holes in the rocker arm shaft deliver lube oil to the lifters for lubrication during operation. The inadequate lube oil flow to the lifters during the series of PMTs that occurred while the shaft plugs were not installed was later attributed to the collapse of the lifter. The station decided that repairs to the lifter would occur during the overnight hours when the preferred maintenance technicians returned to work so the LCO was exited at 1343. The LCO was again entered at 0300 on May 3, 2024, to repair the lifter. A PMT was completed and the LCO was exited for a final time at 0507.

The licensee established procedure MA-AA-716-010, Maintenance Planning, to provide guidance on requirements and expectations of the Maintenance Planning process. Per step 3.3.7 of this procedure, maintenance planners should have identified the correct parts and materials to support the work activities and incorporated this information into the work package. Step 4.6.4 of this procedure states, in part, while developing work instructions, determine repair technique and review the following inputs and others as may be required. Of these inputs, the step specifically lists maintenance history including other Exelon

[Constellation] sites, design specifications, controlled documents, and design documents as items to review. Contrary to this, maintenance planners failed to incorporate information from all available inputs, including drawing KSV-25-4 and maintenance history from Braidwood.

Corrective Actions: In addition to installing the pipe plugs and replacing the damaged lifter, the licensee positively identified the drawing that shows the plugs, the purchase and stocking information of the plugs, and similar work from Braidwood station that was performed in the recent past showing the inclusion of the plugs in their work package. The rocker arm shaft and pipe plugs will be crossed referenced in the licensees database to ensure the connection between the two is not overlooked in the future.

Performance Assessment:

Performance Deficiency: The inspectors determined that the failure to establish work instructions appropriate to the circumstances for performing maintenance on the 2A EDG 7R rocker arm assembly was not consistent with licensee procedure MA-AA-716-010 and TS 5.4.1.a and was a performance deficiency. Specifically, during planning for the rocker arm replacement, maintenance planners failed to identify and incorporate the plugs on drawing KSV-25-4 into the work instructions resulting in insufficient lube oil flow to the lifters that subsequently caused one lifter to collapse.

Screening: The inspectors determined the performance deficiency was more than minor because it was associated with the Procedure Quality attribute of the Mitigating Systems cornerstone and adversely affected the cornerstone objective to ensure the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Specifically, the inspectors determined that the licensees failure to install the plugs in the rocker arm shaft and run the EDG without proper lube oil flow contributed to the collapse of the 7R cylinder lifter, and, as such, had an adverse impact on the 2A EDG reliability. Additionally, the ensuing maintenance to diagnose the issues and perform repairs resulted in additional unavailability time of the 2A EDG.

Significance: The inspectors assessed the significance of the finding using IMC 0609 Appendix A, The Significance Determination Process (SDP) for Findings At-Power. The inspectors screened the issue using IMC 0609, Appendix A, Exhibit 2, Mitigating Systems Screening Questions, and determined that this finding was of very low safety significance (Green).

Cross-Cutting Aspect: H.5 - Work Management: The organization implements a process of planning, controlling, and executing work activities such that nuclear safety is the overriding priority. The work process includes the identification and management of risk commensurate to the work and the need for coordination with different groups or job activities. Specifically, maintenance planners had controlled drawing KSV-25-4 available to them while planning as well as comparative work history from Braidwood but failed to utilize this information while planning for the replacement.

Enforcement:

Violation: Byron Unit 2 Technical Specifications 5.4.1, requires, in part, that written procedures shall be established, implemented, and maintained covering the applicable procedures recommended in Regulatory Guide 1.33, Revision 2, Appendix A, February 1978.

NRC Regulatory Guide 1.33, Revision 2, Appendix A, Section 9 addresses Procedures for Performing Maintenance and Section 9.a states, in part, Maintenance that can affect the performance of safety-related equipment should be properly preplanned and performed in accordance with written procedures, documented instructions, or drawings appropriate to the circumstances.

The licensee established WO 5282402, task 1, major revision 4, on April 22, 2024, to replace the 7R exhaust rocker arm assembly.

Contrary to the above, following discovery of the additional maintenance needed through the start of the PMTs on April 29, 2024, the licensee failed to establish a work package to address the requirements of Regulatory Guide 1.33, Appendix A, Section 9.a. Specifically, maintenance planners failed to identify and include necessary replacement parts of the rocker arm assembly as depicted in drawing KSV-25-4 when establishing work instructions for maintenance on the safety-related EDG under WO 5282402. This ultimately led to improper flow of lube oil within the 7R cylinder causing the collapse of a rocker arm lifter.

Enforcement Action: This violation is being treated as a non-cited violation, consistent with Section 2.3.2 of the Enforcement Policy.

Observation: 0B Diesel Driven Fire Pump (0FP03PB) Performance Issues 71152S The inspectors reviewed the licensees corrective action program to identify potential trends in the performance and system health of the diesel fire pump that might be indicative of a more significant safety issue. The inspectors observed several potential trends related to the equipment health of the diesel fire pump including fuel dilution as documented in Issue Report (IR) 4774816, head gasket condition as documented in IR 4774406, high discharge pressure as documented in IR 4776335, and unexpected loss of power alarms as documented in IR 4782149.

The inspectors performed a detailed review of IRs associated with IR 4774816 0FP03PB Diesel Engine Fuel Dilution Negative Trend, IR 4774406 0FP03PB Diesel Engine Head Gasket Negative Trend, IR 4776335 0B Fire Pump High Discharge Pressure and IR 4782149 0B Fire Pump Loss of Power Alarm.

Specific CAP entries reviewed by the inspectors included, but were not limited to:

  • IR 4765136 0FP03PB Diesel Crank Case Oil Low Viscosity
  • IR 4772609 0FP03PB Diesel Confirmatory Oil Sample Results
  • IR 4774816 0FP03PB Diesel Engine Fuel Dilution Negative Trend
  • IR 4754138 0B Fire Pump Discharge Pressure High During Monthly Run
  • IR 4688979 0B FP Pump Unexpected Alarm While a negative trend related to lube oil samples seems apparent, the station has been taking global actions on improving their processes and procedures for oil sampling and analyses to arrest the trend.

In accordance with corrective action program requirements, the licensee continues to review preventive maintenance strategies and activities for possible enhancements or modifications.

Engineers remain cognizant of the differences between past performance of the old pump and current performance of the new pump to ensure operability is maintained. Identified discrepancies are promptly entered into the corrective action program and addressed with an appropriate level of sensitivity commensurate with the safety significance of the equipment.

No findings or violations of NRC requirements of more than minor safety significance were identified by the inspectors in the course of this review.
