On June 28, 2024, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Byron Station and discussed the results of this inspection with Shane Harvey, and other members of your staff. The results of this inspection are documented in the enclosed report.
One finding of very low safety significance (Green) is documented in this report. This finding involved a violation of NRC requirements. We are treating this violation as a non-cited violation (NCV) consistent with Section 2.3.2 of the Enforcement Policy.

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting a Phase 2 Post-Approval License Renewal at Byron Station, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. [H.3] - Change Management 71003 The inspectors identified a Green finding and associated Non-cited Violation (NCV) of the Byron Station Unit 1 Operating License Condition 2.C.(23)(b)1. Specifically, the licensee failed to revise the procedures for planning insulation repairs prior to April 30,2024, to ensure that the repairs are performed in accordance with specification requirements to prevent water intrusion into the insulation, as described in the License Renewal UFSAR Supplement.

INSPECTION SCOPES
Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

- TEMPORARY INSTRUCTIONS, INFREQUENT AND ABNORMAL
===71003 - Post-Approval Site Inspection for License Renewal The NRC conducted a Phase 2 Post-Approval Site Inspection for License Renewal in accordance with the License Renewal Inspection Program (LRIP). Per Inspection Manual Chapter 2516, the LRIP is the process used by the NRC staff to verify adequacy of aging management programs (AMPs), Time-Limited Aging Analyses (TLAAs), and other activities associated with an applicants request to renew an operating license of a commercial nuclear power plant for an additional 20 years beyond the original operating license period, as stipulated in Title 10 of the Code of Federal Regulations (CFR), Part 54, Requirements for the Renewal of Operating Licenses for Nuclear Power Plants. This Phase 2 inspection was performed before the period of extended operation (PEO), which begins after midnight on October 31, 2024, for Unit 1, and on November 6, 2026, for Unit 2.
The inspectors assessed the licensees implementation and completion of the programs and activities described in regulatory commitments, Updated Final Safety Analysis Report (UFSAR)supplement program descriptions, TLAAs, and license conditions. They reviewed at least 70 percent of the available samples, with this percentage derived from considering the total samples available from both units. However, although most samples apply to both units, the review focused solely on the licensees implementation of license renewal activities in Unit 1 and did not include samples specific to Unit 2. A subsequent Phase 2 inspection may be conducted in accordance with the LRIP, which may review the licensees implementation of license renewal activities in Unit 2, as deemed necessary by regional management.

Failure to Implement External Surfaces License Renewal Commitments as Described in the UFSAR.

[H.3] - Change Management 71003 The inspectors identified a Green finding and associated Non-cited Violation (NCV) of the Byron Station Unit 1 Operating License Condition 2.C.(23)(b)1. Specifically, the licensee failed to revise the procedures for planning insulation repairs prior to April 30,2024, to ensure that the repairs are performed in accordance with specification requirements to prevent water intrusion into the insulation, as described in the License Renewal UFSAR Supplement.
Description:
Condition 2.C.(23)(b)1 of the Byron Unit 1 Renewed Facility Operating License requires the licensee to implement AMPs by April 30, 2024. One of these AMPs is the External Surface Monitoring of Mechanical Components, a new program intended to manage the aging effects of metallic and elastomeric materials through periodic visual inspection of external surfaces for evidence of loss of material and cracking. This AMP is described in License Renewal UFSAR Supplement Appendix F, Section A.2.1.23. This section states:
The procedures for planning insulation repairs will be revised to document that insulation repairs are performed in accordance with specification requirements (e.g., seams on the bottom, overlapping seams) so as to prevent water intrusion into the insulation.
A similar statement is included in Commitment 23 of UFSAR Appendix F, Section A.5.0, Table A.5-1, License Renewal Commitment List.
Water intrusion into the insulation is undesirable as it can lead to undetected corrosion and degradation of SSCs in systems such as Auxiliary Feedwater, Safety Injection, and Service Water, potentially resulting in the failure of the SSC to perform intended functions.
The inspectors identified the specifications requirements to prevent water intrusion into insulation were not included on any active procedure for planning insulation repairs. A preliminary review by the licensee noted the specifications requirements were initially incorporated into Revision 10 of procedure MA-AA-716-010-1010, Maintenance Planning, Work Package Enhancement (MMC) Maintenance Modification Contractor. However, the specification requirements were removed in 2018 with Revision 11 and are not included in the current Revision 12, which became effective in 2020.
Corrective Actions: The licensee entered the deficiency into the corrective action program and is evaluating the actions required to restore compliance.

Performance Assessment:
Performance Deficiency: The licensees failure to revise, prior to April 30, 2024, the procedures for planning insulation repairs to document that insulation repairs are performed in accordance with specification requirements so as to prevent water intrusion into the insulation was contrary to Section A.2.1.23 of UFSAR Appendix F, and License Renewal Commitment Number 23 described in UFSAR Appendix F, Section A.5.0, Table A.5-1, and was a performance deficiency.
Screening: The inspectors determined the performance deficiency was more than minor because if left uncorrected, it would have the potential to lead to a more significant safety concern. Specifically, the licenses failure to revise procedures for planning insulation repairs to prevent water intrusion into insulation would have the potential to result in deterioration of SSCs beneath the insulation which can subsequently lead to their failure to perform their safety functions.
Significance: The inspectors assessed the significance of the finding using IMC 0609 Appendix A, The Significance Determination Process (SDP) for Findings At-Power. The inspectors assessed the significance of the finding using IMC 0609 Appendix A, The Significance Determination Process (SDP) for Findings At-Power. The inspectors answered NO to all the screening question of the Mitigating Systems Exhibit 2. Specifically, the inspectors determined that this finding is of very low safety significance (Green) because the inspectors did not find any example of degraded conditions associated with this performance deficiency.
Cross-Cutting Aspect: H.3 - Change Management: Leaders use a systematic process for evaluating and implementing change so that nuclear safety remains the overriding priority.
Specifically, during a procedure revision, the licensee inadvertently deleted the required commitment language because a systematic process for ensuring the retention of such language was not utilized.
Enforcement:
Violation: License Condition 2.C.(23)(b)1 of Byron Station Unit 1 Renewed Facility Operating License No. NPF-37 states, The License Renewal UFSAR Supplement describes certain programs to be implemented and activities to be completed prior to the PEO. Additionally, it states, The licensee shall implement those new programs and enhancements to existing programs no later than April 30, 2024.
Appendix F of the UFSAR (aka, the License Renewal UFSAR Supplement), Section A.2.1.23, External Surface Monitoring of Mechanical Components AMP, and Commitment Number 23 from UFSAR Appendix F, Section A.5.0, Table A.5-1, License Renewal Commitment List, both state The procedures for planning insulation repairs will be revised to document that insulation repairs are performed in accordance with specification requirements (e.g., seams on the bottom, overlapping seams) so as to prevent water intrusion into the insulation.
Contrary to the above, the licensee failed to implement the new External Surfaces Monitoring of Mechanical Components program prior to April 30, 2024. Specifically, the licensee failed to revise procedures for planning insulation repairs to document that insulation repairs are performed in accordance with specification requirements so as to prevent water intrusion into the insulation as described in Section A.2.1.23 and Table A.5-1 of UFSAR Appendix F.
Enforcement Action: This violation is being treated as a non-cited violation, consistent with Section 2.3.2 of the Enforcement Policy.
Minor Violation 71003 Failure to Implement License Renewal Commitment to Monitor Water Chemistry as Described in the UFSAR.
Minor Violation: The licensee failed to implement a program enhancement and commitment to perform periodic sampling, analysis, and trending of water chemistry for the essential service water makeup pumps engines glycol-based jacket water system. These activities were intended to verify the effectiveness of water chemistry control in mitigating aging effects.
This failure was contrary to UFSAR Appendix F, Section A.2.1.12, Closed Treated Water Systems AMP, and License Renewal Commitment Number 12 from UFSAR Appendix F, Section A.5.0, Table A.5-1, License Renewal Commitment List. Consequently, it was a performance deficiency.
Screening: The inspectors determined the performance deficiency was minor. Specifically, the licensee provided information regarding additional preventative maintenance activities scheduled during the PEO. These activities included periodic visual inspections of the jacket water heat exchangers and non-destructive examinations of the jacket water expansion tanks. These measures would enable the licensee to monitor the aging of the essential service water make up pumps engines.
Enforcement:
This failure to comply with the Byron Station Unit 1 Renewed Facility Operating License No. NPF-37, License Condition 2.C.(23)(b)1 constitutes a minor violation that is not subject to enforcement action in accordance with the NRCs Enforcement Policy.
Minor Violation 71003 License Renewal Commitment Not Implemented for Lubricants in All Pressure Retaining Bolted Joints Minor Violation: The licensee failed to implement a license renewal commitment to prohibit the application of lubricants that contain molybdenum disulfide (MoS2) on pressure retaining bolted joints. This was contrary to UFSAR Appendix F, Section A.2.1.9, Bolting Integrity AMP, and License Renewal Commitment Number 1 from UFSAR Appendix F, Section A.5.0, Table A.5-1, License Renewal Commitment List. The commitment stated, Prohibit the use of lubricants containing molybdenum disulfide on pressure retaining bolted joints. The licensee revised Procedure MA-AA-736-600, Torquing and Tightening of Bolted Connections, to incorporate this commitment in Step 3.2.15, which stated that lubricants containing MoS2 may result in stress corrosion cracking of bolted joints and therefore shall not be applied on structural bolts. The procedure scope addressed structural bolting but did not also include the committed pressure retaining bolted joints as described in the License Renewal UFSAR Supplement. The inspectors noted that maintenance procedures for the reactor head studs and containment air locks included the committed prohibition, but these procedures did not extend to all pressure-retaining bolted joints within the scope of license renewal. Consequently, the failure to implement the committed prohibition to all pressure retaining bolted joints within the scope of license renewal was a performance deficiency.
Screening: The inspectors determined the performance deficiency was minor. While the licensee did not prohibit the use of MoS2 lubricant to all pressure-retaining applications, they had some controls in place. Additionally, the inspectors did not find any instances of the prohibited lubricant being used in pressure-retaining bolting applications within the scope of license renewal.
Enforcement:
This failure to comply with the Byron Station Unit 1 Renewed Facility Operating License No. NPF-37, License Condition 2.C.(23)(b)1 constitutes a minor violation that is not subject to enforcement action in accordance with the NRCs Enforcement Policy.
Minor Violation 71003 License Renewal Commitment Not Adequately Implemented for Spent Fuel Pool (SFP)
Bridge Crane Periodic Inspection Minor Violation: The licensee failed to implement a license renewal commitment to perform periodic inspection of SFP Bridge Crane rails for wear and corrosion and the associated rail clip bolts for evidence of loss of preload. This was contrary to UFSAR Appendix F, Section A.2.1.13, Overhead Heavy Load and Light Load Handling Systems AMP, and License Renewal commitment number 1 from UFSAR Appendix F, Section A.5.0, Table A.5-1, License Renewal Commitment List. The commitment stated, in part, Consistently include
[] rails for loss of material due to wear and corrosion, and bolted connections for evidence of loss of preload. The licensee intended to implement this commitment in procedure 0BMSR FH-5, Spent Fuel Pool Bridge Crane Monthly/Yearly Inspection. However, the inspectors identified that the procedure did not incorporate these inspections; instead, it only required inspecting the bridge for visual cracks in wheels and tracks and checking bolts and nuts for looseness. Consequently, it was a performance deficiency.
Screening: The inspectors determined the performance deficiency was minor. Although the procedure did not include instructions to inspect for the specific aging mechanisms mentioned in the commitment, it did outline routine inspections that could reasonably identify these mechanisms. Additionally, the inspectors performed a walkdown of the SFP area and did not identify any concerns.
Enforcement:
This failure to comply with the Byron Station Unit 1 Renewed Facility Operating License No. NPF-37, License Condition 2.C.(23)(b)1 constitutes a minor violation that is not subject to enforcement action in accordance with the NRCs Enforcement Policy.
Observation: Unclear Procedures and Specifications for High-Strength Bolts 71003 The inspectors identified a process vulnerability in multiple AMPs related to controlling high-strength bolts (i.e., bolts with an actual measured yield strength of 150 ksi or greater).
The License Renewal UFSAR Supplement notes that several AMPs were enhanced to prohibit the use of high strength bolting for various applications. Examples of these AMPs include Reactor Head Closure Stud Bolting, Bolting Integrity, and the AMPs associated with ASME Section XI, Subsections IWF and IWE. The Byron aging management basis documents explain that this prohibition is intended to prevent stress corrosion cracking and bolting failures.
However, the inspectors found that the procedures for controlling high-strength bolting materials were unclear. For example, procedure SM-AA-300-1001 describes procurement engineering roles and, states consider the impact on License Renewal and Aging Management Programs during procurement. Other procedures provide generic guidance, such as consider license renewal requirements or contact the aging management coordinator. In contrast, industry guidance recommends processes like destructive testing or non-destructive testing to determine yield strengths beyond 150 ksi, or alternatively, enhanced monitoring or inspection for installations with high-strength bolts. Interviews with work order planners, procurement, strategic, and design engineers revealed lack of understanding of this guidance and its basis.
Additionally, the inspectors observed that Byron warehouses several high-strength bolt materials that must exceed 130 ksi yield strength to meet their material specifications.
However, these specifications do not prohibit exceeding 150 ksi. The difference between these specification minimums and the aging management limits for controlling stress corrosion cracking is relatively small.
The inspectors did not identify any high-strength bolt installations within the scope of license renewal at Byron. Consequently, this is an inspection observation.
Observation: Lack of Age Tracking for Fire Sprinkler Heads 71003 The inspectors noted that the licensee was not tracking the ages of fire sprinkler heads, despite a commitment to replace or test those older than 50 years. License Renewal Commitment 16.1 specifies that sprinkler heads older than 50 years would be replaced or tested according to the National Fire Protection Association (NFPA) Code 25 at the 50-year in-service date and every 10 years thereafter. Since Byron Station is less than 50 years old, no sprinkler heads are currently beyond this age. This observation was documented the licensees corrective action program as IR 04782644, NRC ID: 50 Year Fire Protection Sprinkler Testing Due Date Error.
