13. Environmental Site Visit The resident inspector attended portions of the NRC/TVA environmental site

- visit conducted on June 15, 1982. This visit included participation by NRC, State and TVA personnel. In addition, the resident inspector reviewed the

request for additional information included in NRC letter to H. G. Parris from E. G. Adenson dated June 8, 1982 (Docket Nos. 50-438/439).

14. Hanger Inspection During the week of June 14, 1982, the resident inspector performed a detailed inspection of two (2) seismic supports on the Decay Heat Removal System. The two hangers inspected were IND-MPHG-0048 Rev. 2 and IND-MPHG-0049 Rev. 3. Hanger #1ND-MPHG-0048 Rev. 2 was identified as having the following deficient condition: ITT Grinneli Sketch No. IND-MPHG-0048 Re sheet 1 of I shows the embedment plate (P 936) as a standard flat surface embedment plate. However, the as-built condition, as shown on TVA drawings 4RW0517-X2-04 Rev. 7, 4RW0518-X2-10 Rev. A, and 4RWO518-X2-12 Rev. 7, includes four (4) 1 3/4" dianeter rods threaded 5" on each end penetration 4" through the embedment plate (P936). Hanger IND-MPHG-0048 Rev. 2 was installed with its i" x 4" x 4" structural tubing over the exposed rod thus preventing the installation of a nut and washer as required by TVA drawing No. 4RWO518-X2-12 Rev. This hanger was completed and signed off by Q The above condition is identified as unresolved item 50-438/82-19-03 " Hanger Embedment Plate."

15. Clarification of Previous Reports Report 438,439/82-13 In paragraph 8 (page 13) change (closed) title from Inspector Followup Item (IFI) 438,439/80-08-06 to Licensee Identified Item (LII) 438, 439/80-08-06 In the second part of paragraph 8 (page 13) change the wording from " numbered MEB 8007" and "NCR BLN MEB 8007" to " numbered MEB 80-07" and "NCR BLN MEB 80-07", respectivel . Power Production Calibration of Equipment During portions of Unit 1 make up and purification system flush through pump INV-MPMP-002, the resident inspector observed four Power Production personnel calibrating a temporary pressure switch. During this calibration effort two potentially deficient conditions were identified. The cali-bration equipment used was not being maintained in a clean condition based on the level of cleanliness required for the system in which the calibration was being performed (i.e. level A, B, clean) and a lack of knowledge was noted on the remaking of a compression fitting after calibration. Power production personnel could not correctly answer the question; when remaking a compression fitting, af ter calibration, how tight do you retighten the fitting? Two answers were given and both were incorrect (1st answer - 11 turn after hand tight, 2nd answer - i turn after hand tight). The correct answer is specified in the Parker Henninfin catalo r

Additional investigation of Power Production revealed the following: no procedures exist which specify maintaining cleanliness controls on calibra-tion equipment. The above is identified as Unresolved Item 438,439/82-19-02, " Power Production Calibration of Equipment."
