Exit Interview The inspection scope and findings were summarized on January 8,1982,- with -

those persons indicated in paragraph 1 above.

Licensee Action on Previous Inspection Findings (0 pen) Violation (438/81-30-01 and 439/81-30-01) - Improper curing and testing of concrete cylinders.

The entire surfaces of concrete test cylinders 5554A through 5557A and 5554B through 55578 were not being main-

.tained in a moist condition in the curing room as specified by-G-2 and ASTM C-31.

The rate of loading being applied -to concrete test cylinders exceeded the 20-50 psi per second rate specified in G-2 and ASTM C-39.

The.

inspector. examined the curing room and obse'rved compression testing on concrete test cylinders. Curing of test cylinders is being done' in accord-

'

ance with requirements. Observation of compression testing indicated' that i

- the rate of_ loading is still being ~ applied at a rate slightly in excess of the specified 20 -' 50 psi.per second loading rate (50 to' 55 psi per second).

This slightly higher rate of loading would have less than a one percent affect on test results. This item remains open pending NRC's receipt of.the-

Licensee Identified Items (10 CFR 50.55(e))

(0 pen) Item (438/81-04-03, 439/81-04-03) - Buttonheads on rock anchorheadt.

Inspection of-the rock anchor tendon anchorheads in Unit 1 indicated some of the tendon wire buttonheads may not be properly seated on the anchorhead.

TVA initially reported this item on September 23,.1980.

Interim reports

. addressing raised.buttonheads were submitted to NRC on. 0ctober 23, 1980, December 29,-1980, April 28, 1981 and October 14, 1981. The next report is to be submitted to NRC by ' January 14, 1982.

(0 pen) Item-(438/81-20-02, 439/81-20-02) - Dimensionally nonconforming anchorage material.

Dimension checks on machining of anchorage ' material disclosed that some anchorage materials are oversized and some are under-sized. TVA is continuing their evaluation and will _ submit their next report to NRC by' January 22, 1982.

Applied pipe anchor support loads are' greater than the assumed pipe anchor support loads used in the design of the supporting wall.

TVA is continuing their evaluation of this item and will submit their next report to NRC by February 24, 1982.

-(0 pen)-Item (438/81-20-05) - Lockoff pressure on postensioning system.

A recheck of the lift-off stresses in the Unit I tendons disclosed a signi--

ficant loss of prestress force in several tendons. TVA is conducting tests on the postensioning system and will submit a report to NRC.upon completion-of testing.
