licensee identified items (10 CFR 50.55(e)) (Units 1 and 2), inspector follow-up items (Units 1 and 2), safety-related piping (Units 1 and 2), reactor coolant pressure boundary piping (Units 1 and 2) and safety-related structures (Unit 1).

Results Of the six areas inspected, no violations were identified in five areas; one violation was found in one area (Violation - Installed Piping Cleanliness -

paragraph 8). No deviations were found.

Licensee Action on Previous Inspection Findings I

(Closed) Violation (438/80-27-01, 439/80-26-01): Failure to provide pres-l surizer surge line grind procedure. This violation involved the licensee's (

failure to include instructions for grinding and inspection of the inside diameter (ID) of a piping weld, where this grinding was required to meet i

design requirements specified by drawing. The licensee's letter of response

to this violation,-dated January 30, 1981,-has been reviewed and determined acceptable by Region II.

In its letter of response, the licensee indicated that instructions would be provided for the required grinding in a Sequence Control Chart (SCC) to be prepared for the work. The inspector found that,

subsequently, the licensee had determined that grinding of the originally I

questioned weld was unnecessary.

However, an' ID grinding requirement was added for another weld in the same line grinding was specified for the 10 of a

the first surge line piping weld away from the pipe to safe-end weld. The inspector verified the SCC's had been prepared for grinding and inspection of this weld on both Units and considers the violation closed.

The inspector also asked to see the reports for visual and penetrant (PT)

examinations required on the ground welds.

The licensee was unable to provide the reports and the inspector questioned whether the examinations had been performed.

Licensee personnel initiated an investigation to determine the status of the inspections and reports. The inspector identi-fled his concerns for the missing reports and possibly bypassed inspections as unresolved item 438,439/82-11-02, "PT and Visual Examination of Ground Weld" 4.

Unresolved Items Unresolved items are matters about which more information is required to determine whether they are acceptable or may involve violations or devia-tions. A new unresolved item identified during this inspection is discussed in paragraph 3.

Licensee Identified Items (10CFR50.55(e) Items) (Units 1 and 2)

The inspector reviewed licensee identified items (LII's) with cognizant site personnel as described below, a.

Insufficient Information The inspector found that the majority of the licensee's final reports submitted for LII's did not provide sufficient information to permit analysis and evaluation of the deficiency and of the corrective action, and that relevant data was not readily available at the site. Typi-cally, the missing information specifically pertained to how and by whom (functionally) the deficiency was discovered, identification of the root cause, assurance that the full extent of the deficiency has been determined, and the adequacy of the action to preclude recurrence.

The inspector discussed and requested such information from site personnel for the following items:

Items Reviewed and Closed As described below, the inspector reviewed three items and determined

that they could be closed.

(1) (Closed) LII (438,439/81-01-02):

Deficient flange computer report.

The licensee submitted its final report on this item to Region II in a letter dated October 22, 1980.

The inspector reviewed the deficiency and corrective action, as described in the final report and the licensee's Nonconformance Report (NCR) BLN CEB 8006. The inspector also verified changes to the licensee's Flange Report CEB-MEl-77-5, which were required for implementation of the corrective action. The item is closed.

(2) (Closed) LII (438/81-16-10): Sodium hydroxide storage tank.

This item was reported to Region II as a potential deficiency in accordance with 10 CFR 50.55(e) on October 6,1980. In subsequent evaluation, the licensee determined that the item was not report-able.

The inspector reviewed the licensee's documentation and evaluation of this item on NCR 1291 and the worksheet used to determine its reportability.

The inspector concluded that the item could be closed.

l (3) (Closed) LII (438,439/81-16-11): Hanger weld joints not to spect-fication.

This item was reported to Region II as a potential deficiency in accordance with 10 CFR 50.55(e) on January 15, 1981.

In subse-quent evaluation, the licensee determined that the item was not reportable. The inspector reviewed thc licensee's documentation and evaluation of this item on NCR 1332 and the worksheet used to determine its reportability.

The inspector concluded that the item could be closed.

Inspector Fallow-up Items (IFI's) (Units 1 and 2)

a.

(Closed) IFI (439/80-07-07): Bellows repair.

This item was opened by the inspector to follow-up on repair of damage to safety-related metal bellows. The inspector discussed the repair with the licensee's WEU persennel and reviewed documentation of the completed repair contained on the licensee's NCR 923 and APC cards.

The inspector also reviewed the documentation of tests performed to establish the adequacy of such bellows repairs, as contained in Path-ways Bellows Report TYCYC-020 Rev. B.

(Closed) IFI (438,439/81-05-02):

Technique' for reading gas flow meters.

The inspector questioned welders responsible for performing safety-related welding as to their knowledge of reading welding gas flow meters. The inspector is satisfied that their knowledge is adequate.

c.

(Closed)' IFI (438,439/81-16-05):

Evaluation of magnetic pat ticle indications when ASME code criteria does not apply.

The inspector reviewed the licensee's evaluation of magnetic particle indications, as described in NCR 1250, with responsible WEU personnel.

The inspector is satisfied with the licensee's disposition of such indications.

Safety-Related Piping (Units 1 and 2)

The inspector reviewed the licensee's evaluation and acceptance of apparent forming defects in safety-related = piping as documented on WCR 1756.

The review was performed to verify that the evaluation and acceptance was in compliance with regulatory requirements and FSAR commitments-including ASME '

Section III (74S74) code requirements.

The piping involved was 16 inch piping' spool pieces ~1KE8 and 1KE9 (Unit 1) and 2KE8 and 2KE9 (Unit 2). The inspector examined the outside diameter (00) surface of the subject piping, reviewed the NCR disposition, and reviewed guidance relative to the accept-ability of such conditions provided by the licensee'_s design engineer in memorandum MEB 820312023.

No violations or deviations were identified.

8.

Reactor Coclant Pressure Boundary Piping-Observation of Work and Work Activities (Units 1 and 2)

The inspector selectively observed non-welding related work for reactor coolant pressure boundary (RCP8) piping to verify compliance with regulatory requirements and FSAR commitments - including _ the requirements of the applicable code, ASME Section III (74S74).

Examples of. piping work observed by the inspector were as follows:

a.

Traceability identification on stored piping such as serial number 23-Al-2016-50-1 (Unit' 2)

b.

Segregation of RCPB (ASME Section III Class 1) piping such as that identified in a. above and spool piece 2NV-188 (Unit 2)

c.

Temporary support of Unit 2 pressurizer surge line piping d.

Code nameplate identification on welded spool pieces such as nameplate serial number SN2639-208 (Unit 2)

Covering of openings in installed piping, such as open valves adjacent to welds INL0005 (Unit 1) and 2NL0005 (Unit 2), in order to maintain cleanliness The above piping work was observed by the inspector to verify compli-ance with requirements and commitments implemented through the licen-see's process specification 4.M.1.1(h) (storage, marking, handling and cleanliness requirements), procedure QCP1.2R9 (storage requirements),

and procedure QCP6.16R0 (cleanliness control requirements).

The inspector specifically observed whether the work had-been performed in accordance with the above specifications and procedures and whether the specified materials were used. The inspector found that, contrary to the requirements of procedure QCp6.16RC, the valve next to weld 1NL0005 was open und uncovered, permitting dirt and debris to enter this RCPB piping.

This noncompliance with procedure requirements is considered a violation of 10 CFR 50, Appendix B, Criterion V, and is identified as Violation 438/82-11-01, " Installed Piping Cleanliness".

Within the areas inspected, one violation was identified, which is described above. No deviations were identified.

9.

Safety-Related Structures (Welding) - Review of Quality Records (Unit 1)

The inspector selectively reviewed records for welds in safety-related structures located outside the containment to verify compliance with regu-latory requirements and FSAR commitments - including code requirements.

The records reviewed were for three welds identified 1A6008007, 1A600817 and 1A600819.

The code applicable to these welds i s AWS D1.1-77.

The records were reviewed to verify:

Receipt verification of identity and conformance with specification for the welding material (Heat 1022A).

b.

Welder qualifications c.

Welding inspector qualifications No violations or deviations were identified.
