Containment (Prestressing) - Review of Quality Records - Unit 1 The inspector examined quality records for restressing of the 113 FWC horizontal tendons in unit one.

Restressing of these tendons was recom-mended when a recheck of the original lift-off stresses disclosed a significant loss of pretress force. The loss of pretress force was reported by the licensee to NRC on July 17, 1981 and is being tracked under Licensee Identified Item number 438/81-20-05. Acceptance criteria examined by the inspector appear in the following documents:

FCR-033, Restressing of 113 Unit One FWC Tendons Examination of stressing cards for the 113 FWC horizontal tendons showed that restressing was done in accordance with accepteble criteria. Evalu-

Licensee Identified Items (10 CFR 50.55(e))

Closed Item (438/81-04-03, 439/81-04-3) - Buttonheads On Rock Anchorheads.

Inspection of the rock anchor tendon anchorheads in Unit One indicated some of the tendon wire buttonheads may not be properly seated on the anchorhead.

TVA initially reported this item on September 23, 1980.

Interim reports addressing raised buttonheads were submitted to NRC on October 23, 1980, December 29, 1980, April 28, 1981 and October 14, 1981.

The inspector examined TVA's final report dated January 18, 1982. Inspection of each rock anchorhead in Unit One and Unit Two (185 rock anchorheads in each unit)

indicated that there were no wire failures and that the raised button heads ~

were oversized button heads that were effectively seated on adjacent buttonheads. This item is closed.

(0 pen) Item (438/81-20-02, 439/81-20-02) - Dimensionally nonconforming anchorage material.

Dimension checks on machining of anchorage material disclosed that some anchorage materials are oversized and some are under-sized. TVA is continuing their evaluation and will submit their next report to NRC by April 15, 198...,

Applied pipe anchor. support loads are greater than the assumed pipe anchor support loads used in the design of the supporting wall.

TVA is continuing their evaluation of this item and will submit their next report to NRC by April 22, 1982.

(0 pen) Item (438/81-20-05) - Lockoff pressure on postensioning system.

A recheck of the lift-off stresses in the Unit 1 tendons disclosed a significant loss of prestress force in several tendons. TVA is conducting

tests on the postensioning system and will submit a report to NRC. upon completion of testing.
