i APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority Docket No. 50-438 Bellefonte 1 License No. CPPR-122 As a result of the inspection conducted April 26-29, 1982, and in accordance with the NRC Enforcement Policy, 47 FR 9987 (March 9,1982), the following violation was identified.
10 CFR 50, Appendix B, Criterion V, as implemented by FSAR Section 17.A1.5, requires that activities affecting quality be prescribed and accomplished in accordance with documented procedures or instructions. Bellefonte pro-cedure QCP 6.16RO requires that openings on piping materials and equipment be sealed or capped to maintain cleanliness.
Contrary to the above, piping installed in the core flooding system was not adequately sealed or capped to assure maintenance' of cleanliness in that, on April 27, 1982, the valve adjacent to weld 1NL0005 in the core flooding system was open and dirt and debris had entered the system piping through the opening.
