Watkins, Region 11 Discussion Proposed Examinations The proposed written, walkthrough, and simulator examinations were effective in testing the indiv: dual weak areas identified on the previous requalification examination Some critical tasks on the proposed simulator examinations needed to be modified. The events as written did not require any actions which met all of the criteria for a task to be critical. Four questions on the proposed written examination were changed in order to make the distractors more plausible, Operator Performante During the simulator examination the crew failed to recognize trehds prior to the sounding of the annunciator alarms. However, the crew's recognition of off-normal trends and status was timely and appropriate when the alarms sounde .. . .

Report Details 2 Exit Meeting At the conclusion of the site visit, the examiners met with representa-tives of the plant staff indicated in paragraph 1 to discess the results of the examinations and inspection findings. This exit was conducted in conjunction wi+h the initial examination exit that was given during the week of October 19, 1992.
