NRC INSPECTION REPORT 50-397/96-01 AND NOTICE OF VIOLATION This refers to the inspection conducted by Mr. T. 0.

McKernon and others of this office on March 25-28.

1996. at the Washington Nuclear Project-2 facility.

The purpose of the inspection was to determine whether activities authorized by the license were conducted safely and in accordance with NRC requirements.

At the conclusion of the inspection; the tindings were discussed with Mr. G. Smith and those members of your staff identified in the enclosed report.

In a telephone conversation on April 10, 1996.

between Messrs, T. McKernon and R. Barr of the NRC staff and Ms. Fernandez, who stated your staff's position with regard to the apparent violation.

This position, was contrary to the NRC's position and is further discussed in this report.

Areas examined during the inspection are identified in the report.

Within these areas.

the inspection consisted of an evaluation of the qualifications of applicants for senior reactor operator licenses and observation of activities in the control room and within the plant.

The evaluation included an eligibility determination and the administration of comprehensive written examinations and operating tests.

This initial operator examination was administered as part of a pilot examination process in accordance with Generic Letter 95-06.

"Changes in the Operator Licensing Program."

The results of this inspection are documented 'in the enclosed report.

Appropriate licenses have been issued to the two applicants who successfully completed thei r examinations.

Our inspection determined that improvements in the control room conduct of operations have continued to be achieved since the observations noted in the June 1995 special inspection.

These improvements were noted in crew communications.

crew briefings.

command and control practices.

procedural usage and adherence.

the control room operating environment.

and in-plant walk throughs during the performance of equipment operator duties.

However, based on the results of this inspection.

the NRC has determined that a violation of NRC requirements occurred.

This violation is cited in the enclosed Notice of Violation (Notice) and the circumstances surrounding it are described in detail in the subject inspection report.

The violation is of concern because s>milar recent events have occurred which represent a loss of control of safety-related work activities and corrective actions to these previous events could have prevented the inadvertent half-scram actuation event which occurred on March 27.
