Subject: NOTICE OF VIOLATION (NRC INSPECTION REPORT N0. 50-373/86040)

This refers to the inspections conducted during the period of October 8 through November 17, 1986, of activities at LaSalle County Nuclear Station, Units 1 and 2, authorized by NRC Operating Licenses No. NPF-11 and No. NPF-18. These inspections identified two examples of a procedural violation.

The violations took place on October 16 and 17, 1986, when a required second verification of a valve position was performed improperly and an off-normal indication existed in the control room and was not recognized.

These matters were discussed on February 13, 1987, during an enforcement conference held at the NRC Region III office between Mr. L. DelGeorge (CECO)

and Mr. C. E. Norelius (NRC) and members of their staffs.

As stated in our correspondence to you dated January 21, 1987, the two procedural violations were under consideration for escalated enforcement action. As a result of the meeting on February 13, 1987, we have decided that escalated enforcement action is not warranted at this time and we are combining the two violations and issuing one Severity Level IV violation.

However, further violations similar to these could result in escalated enforcement action.

You are required to respond to this letter and should follow the instructions specified in the enclosed Notice when preparing your response.

In your response, you should document the specific actions taken and any additional actions you plan to take to prevent recurrence.

We have already received your letter regarding corrective action completion dates dated February 27, 1987, and the summary handouts from your presentation at the enforcement conference. However, because some of the dates for actions have changed and in order to have a complete description of all your corrective actions and implementation dates we are requiring a response to this letter as noted above.
