The inspector stated that the inspection had been scheduled to review the reactor internal fit up procedures and observe the trial fitup of the reactor internals.
Due to conditions

and problems encountered with the Polar Crane and documented by TECO the inspection of the reactor internals trial fitup was not possible.
He further indicated that he had i
reviewed NCR 870 that had been issued on the Polar Crane indicating that the main hook had not been NDE examined as required by ANSI-B30.2 requirements.,. The inspector added that due to the fact that neither TECO or their contractors, Babcock and Wilcox or Bentley, had documented approved pro-cedures to cover the inspection testing and certification of lif ting equipment as required by 10 CFR, Part 50, Appendix B, Criterion V, an infraction in regard to this item would be issued.
Since the use of the equipment had been curtailed an NCR issued and procedures preparation was in progress the inspector stated that no reply to this item would be required.

necessary procedures to prevent this condition from reocurring.

Reactor Vessel Cleanliness Inspection December 22-23, 1975 The inspectors reviewed Babcock and Wilcox Field Construction Procedure No. 156, Revision 0, Approved 12-8-75, Titled " Clean reactor vessels." During the visual inspection the inspectors
noted that the vessel had not been made ready for trial fitup of the reactor internals, scaf folding, cables, and other items were still in place in the vessel.

Polar Crane Test and Inspection

The incpectors requested documentary evidence that the Polar Crane had been inspected prior to a trial fitup of the reactor internals.
Monthly visual inspection records were available and reviewed.
llowever, no records were available to indicate that the crane had been inspected and tested as required by ANSI B30.2.0-1967, Chapccr 2-2, " Inspection Testing and Ibintenance of Cverhead and Gantry Cranes."
In addition, there were no instructions available to control the inspection of the crane.
On December 12, 1975, a Nonconformance Report No. 870 was generated indicating that the hooks of the Polar Crane had not received a magnetic particle inspection since Fbrch 3, 1972. As a result of
'
the NCR,. the 180 Ten polar crane hook S/N 8E770-P1 was magnetic particle tested. The test en December 22, 1975, showed that indications detected in the hook during an earlier inspection were still present in the 6" diameter hole however, evaluations of the test results had not been completed.

when manufactured, however, a nonconformance report was generated, an engineering evaluation made, and the heck was accepted.

The inspectors reviewed a nonconformance report (No. 881) issued on December 29, 1975, as a result of further evaluation of the MT indications in the eye of hook SIN 8E770-Pl. Also revicued were reports of a inspection of the Polar Crane performed by llarneschfeger Corporation on December 30, 1975, certifying the crane for use.
The Polar Crane hook and block assembly was removed and replaced with the 190 ton assembly from the Turbine Building Crane. The hook of

this assembly was MT inspected Ly Testmaster.
The MT report indicated i
that this hook was acceptable for use. The hook and bloch assembly
removed from the Polar Crane is being returned to the llarneschfcger Corporation for repair or replacement.

The inspectors observed the lif ting of the Reactor Internals from the vessel, noting the scribe lines for indication of movenent of the Guide Blocks.
The inspecters determined that work was being performed in accordance with B&W approved procedures.
