Inspection on August 31-September 3,1982.
Areas Inspected This routine, announced inspection involved 23 inspector-hours on site and at the general office in the areas of generator step-up (GSU) transformer failure of August 22, 1982, and the licensee's recommendations and inspector follow-up items that were identified as a result of GSU transformer failure in 1981.
Results Of the areas inspected, no violations or deviations were identified.

Inspection of Generator Step-up (GSU) Transformer Failure on August 22, 1982 An inspection was made as a result of GSU phase-B transformer failure on August 22, 1982. This failure was considered to be caused by a broken bolt from the condenser guide bracket.
The broken portion of the bolt lodged between the condenser and the ground sleeve of the high voltage (HV)
This initiated a fault that eventually caused the tran;former to
,
fail. This type of failure is most unusual and would not be expected to happen in other HV bushing of this type.

A new transformer was installed. Standard electrical tests were performed and oil samples were taken on the remaining GSU transformers. These tests
and samples indicated that the transformers were satisfactory for service.
An ultrasonic corona sensor with readings in pico coulombs was installed to monitor corona on phase A and C transformers.
Readings of this sensing circuit, with plant at approximately 100*f power and various lower power
,
levels, indicate that corona levels are satisfactory.
Within the areas examined, no violations or deviations were observed.

The inspector reviewed the recommendations in the licensee's transformer i

Failure Analysis Report of 1981 which was transmitted to Region II by letter t
of January 8,1982, and the IFI's of Regions II's report 50-339/81-16.
Corrective actions have been implemented or initiated for items in these l
reports. The IFI's with regard to the above are discussed below.

Generic Concerns are Identified and Addressed. The licensee's report

states that there were no generic transformer design or system problems associated with those failures. They made several recommendations as a result of their investigation and have subsequently implemented or initiated actions to put these items into operation.

The scope of the work has been formulated, approved by site personnel, and has been sent to the Budgeting Department.

(Closed) 50-339/81-16-11, Evaluate Need for a Communication System on a

Common Radio Frequency for all Responding Fire Fighting Organizations.
The licensee states that a common radio frequency communication involves the FCC and that it is not feasible or possible to implement this type of communication system.
Since Security Officers must accompany all undesignated vehicles on site, it is considered that the existing system, utilization of security force radios on plant operation channels, will suffice.

(Closed) 50-339/81-16-14, Review Scope of Maintenance Program for Fixed Fire Protection Systems.
The licensee has initiated a seniannual preventive maintenance program for fixed fire protection systems by developing P.M. code M-00-FP/SA-1 and SA-2.
This program checks fire hydrants, and gate valves of the fixed fire protection system including their operability and a brief flush.
