5.
Independent Inspection a.
The inspector examined results of the cleanup and ongoing cepairs to damaged concrete from the Unit 2B main transformer fire of July 3, 1901.
No category I structures were damaged by the fire.
The transformer concrete support pad and transformer have been replaced.
The licensee reported the fire in the Unit 28 transformer as an LER on July 3, 1981 and is currently preparir.] an evaluation report for NRC.
Flaming oil from the fire spilled down on the Unit 3 turbine foundation area but did not appear to have an adverse affect on the existing foundation caissons. Oil spilled on the Unit 3 turbine foundation area is being cleaned up by the licensee.

The inspector examined the concrete batch plant, concrete and soils

testing laboratory and qualifications of civil QC inspectors.
The licensee is currently upgrading the concrete test cylinder curing room and calibration controls on test sieves to meet ASTM standards.
No category I concrete or soils are being placed at this time.
No violations or deviations were identified.
6.
Lakes Dans and Canals - Observation of Work and Work Activities, Unit 3 The inspector examined completed work on the service water reservoir. This facility serves ; nits one, two and three and was completed (except for the Unit 3 pump house) during the construction of Units 1 and 2.
Acceptance cr;teria examined by the inspector appear in the following documents.

FSAR Section 3.8.4 b.
Specification NAS-348, Specification for Clearing General Excavation and Construction of Service Water Reservoir.
Examination of the completed facility showed that the service water reservoir has been constructed in accordance with the FSAR and specification NAS-348.
However, the inspector was not able to verify that the licensee is performing the inservice inspection and monitoring of instrumentation at the frequency specified in FSAR table 3.8.4-3.
Inspection and monitoring of the service water reservoir was identified to the licensee as Unresolved Item 50-404/81-01-01, " Service Water Reservor Monitoring."
No violations or deviations were identified.
7.

The inspector examined preliminary specifications and drawingt for the proposed foundation preparation and testing on the Unit 3 Service Building, Safeguards Building, Mainsteam and Quench Spray Building and Recirculations Spray Pump Shaf t.
