Licensee Action on Previous Inspection Findings (Closed) Violation (80-333/81-27-05) Indoctrination and Training Procedure, ITP-13, " Fire Protection Training," requires that each fire brigade perform a shif t drill or exercise at least quarterly.

But the licensee had no program / system to ensure the operations and security personnel who are on different rotating shifts and make up the fire brigades were included in the required minimum of two drills per year.

The licensee has established a system to monitor and reflect the current Fire Brigade training status. The inspector reviewed the Training Reports generated for the six fire drills, conducted in the first quarter of 1982, t

I to ensure that the training information was documented in the Fire Brigade Training Status Book. No exceptions were noted.

The Inspector reviewed the proposed revision to ITP-13 to ensure the requirements of 10 CFR 50 Appendix R were included in revised procedure.

The licensee's representative committed to issuing ITP-13 by April 1, 1982.

Based on the actions taken by the licensee, this item is closed.

Seven licensed operators did not complete the required review of l

special procedures or abnormal and emergency procedures.

l One licensed operator's written examination was not graded.

--

Systematic evaluations of performance and compe tency for three licensed operators were not performed.

The Resident Inspector reviewed and verified that the required review of special procedures, the systematic evaluations and grading of the written exam were completed by the licensee.

The licensee has established a management control system that will provide a summary and status of the individual licensed operators training. The system consists of an Individual Requalification Training,Sununary depicting each licensed operator's current requalification status, and the Program Administrators' Guide which is a one pa'ge matrix of requalification requirements versus licensed operators.

Concurrently, the Training Coordinator started to perform. quarterly audits of the licen' sed operator requalification program to prevent further recurrence.

Such audits were completed February 2, 1981, and April 14, 1981, but there were no additional audits performed by the Training Coordinator for the remainder of the year.

The licensee's representative acknowledged the inspector's findings and committed to resuming these audits of the Licensed Operator Regular Program starting with the first quarter of 1982.

The inspector reviewed the licensed operator requalification program and found no discrepancies or violations (see paragraph 5). Based on the above determination this item is closed.

3.

General Employee Training a.

Program Administration The inspector reviewed Indoctrination and Training Procedure, ITP-3,

" General Employee Training," Revision 2, November 19, 1980, to determine its conformance with the training requirements of 10 CFR 50, Appendix B, Criterion II; 10 CFR 19.12; 10 CFR 20;_10 CFR 73.50; Technical Specifications, Sections 6.3 and 6.4; and ANSI 18.1-1972, " Selection and Training of Nuclear Power Plant Personnel," Section 5.4, for new I

and existing employees; temporary employees; technicians; and craft I

personnel.

This training provides plant personnel with indoctrination training and periodic retraining in the following areas:

Program Implementation

The inspector randomly selected and examined the licensee training records to verify that the required training has been given.

In L

addition, the inspector conducted seven interviews with individuals

whose records were examined to verify that:

'

The scope of the training was similar to that contained in the license's training records;

--

The training was meaningful to those individuals attending; and

--

The areas presented were covered accurately and sufficiently from the partic.ipants point of view.

The interviews were conducted with craftsmen, technicians, clerical and security personnel, including two female employees.

c.

Findings One violation was identified, failure to provide adequate General Employee Training.

(1)

Indoctrination and Training Procedure, ITP-3, Revision 2, November 19, 1980, Section 3.5.9, General Employee Retraining, requires annual requalification/ retraining in Radiation Protection;-Security Plan and procedures; and Emergency plan and procedures.

Contrary to the above, the inspector found that in thirteen of thirty training files examined that the annual requalification training was not provided in the past 15 months.

ITP-3, Section 3.5.8, requires initial training in Quality Assurance (QA) and Quality Control (QC) for permanent employees within six

weeks of hire.

Contrary to the above, the inspector's review of eight training records for new employees in 1980-1981, indicated that the licensee has not provided the initial QA and QC training for new hires within six weeks.

(3)

ITP-3, Section 3.5.7, Industrial Safety, states, indoctrination in industrial safety is the normal responsibility of each new employee's department superintendent or his direct supervisor at the time of hire.

ITP-3, Section 3.4.1, states the Training Coordinator is responsible for the performance and documentation of training in accordance with this procedure.

Contrary to the above, the inspector determined that Industrial

"

Safety training is not being provided for all new employees at the time of hire and that there is no management control system to ensure that the required training is performed.

,

The licensee's representative acknowledged the inspector's findings and stated that:

--

All overdue requalification training will be scheduled for completion in March 1982; and

--

ITP-3 would be revised and issued by June 1,1982 to incor-porate a management control system to ensure the necessary initial training (QA/QC and Industrial Safety) as well as requalification training is performed.

,

Failure to provide the required training, as discussed above is contrary to Technical Specification 6.4, Retraining and Replacement

,

training, ANSI N18.1-1971 and ITP-3 and constitutes a violation (333/82-05-01).

4.

Craft,;,d Technical Training a.

Pro 1 ram Administration The 'nspector reviewed the following Indoctrination and Training Procedures to verify that the licensee has established a program to provide technical training for non licensed personnel:

No violations were identified.

Implementation The inspector selectively sampled and examined the licensee's training records for craf t and technical personnel.

In addition, interviews were conducted with individuals whose records were examined to verify that the following training was provided:

--

formal training commensurate with the job classification; fire fighting training;

--

--

on the job training to supplement the formal training; and

--

retraining.to maintain proficiency for inspection and testing in accordance with ANSI-N45.2.6-1973.

c.

Findings No violations were identified; however, one inspector concern was identified and will be reviewed in a subsequent inspection.

Indoctrination and Training Procedure 6 and 9 require that each new hire Mechanic, Electrician or Instrumentation and Control Technician, within six months of hire, should be provided with training in plant design.

The inspector determined from the record review and interviews that four new employees were not given the plant design training within six months of hire and were not scheduled for training until October of 1982.

The licensee's representative acknowledged the inspector's findings and committed to the following resolution:

--

Establish a schedule for implementation of the required plant design training, to include all present plant employees and all-new employees by July 1, 1982.

Shifting the plant design training scheduled for October 1982 up

to July 1982 to expedite the required training, and to assure that all licensee personnel will receive the required plant design training by October 31, 1982.

This item will be reviewed during a subsequent NRC:RI inspection (333/

82-05-02).

5.

Licensed Operator Requalification Training a.

Program Administration The inspector reviewed the licensee's requalification program as implemented by Indoctrination and Training Procedure, ITP-5, " Licensed Operator Requalification," Revision 4, August 7,1980, to verify that the program comforms to the requirements of 10 CFR 55, Appendix A; the Accepted Operator Requalification Program - submitted as exhibit B to Amendment 22 to the facility license; and the revised training criteria of the March 28, 1980, letter to all power reactors from H. Denton.

The inspector determined from a review of this procedure that the program as currently established provides for the following:

An established, planned continuing lecture schedule which includes

--

instr'!ction in thermodynamics, heat transfer and fluid flow;

--

documentation of personnel attendance;

'

--

written requalification examinations; i

--

required reactivity control manipulations through simulator

'

training; discussion / reviews and lectures of changes in facility design,

--

procedures and the facility license; review of abnormal / emergency procedures; and

--

systematic performance evaluations.

The inspector interviewed two licensed senior reactor operators, who were a shift supervisor and a staff member, and one licensed reactor operator. The interviews verified the accuracy of the records review.

6.

Management Meetings Licensee Management was informed of the scope and purpose of the inspectior at the entrance interview conducted on March 1, 1982.

The findings of the inspection were periodically discussed with licensee representatives during the course of the inspection. An exit interview was conducted on March 4, 1982 (see paragraph 1 for attendees) at which time the findings of the inspection were presented.
