SEQUOYAH NUCLEAR PLANT, UNITS 1 AND 2 - INTEGRATED INSPECTION REPORT 05000327/2019002 AND 05000328/2019002

Dear Mr. Shea:

On June 30, 2019, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at your Sequoyah, Units 1 and 2. On July 17, 2019, the NRC inspectors discussed the results of this inspection with Mr. Matt Rasmussen and other members of your staff. The results of this inspection are documented in the enclosed report.

One finding of very low safety significance (Green) is documented in this report. This finding involved a violation of NRC requirements. We are treating this violation as a non-cited violation (NCV) consistent with Section 2.3.2.a of the Enforcement Policy.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting a integrated inspection at Sequoyah, Units 1 and 2 in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. [H.5] - Work Management 71111.15 The inspectors identified a green, non-cited violation of 10 CFR 50 Appendix B criterion XVI when the licensee failed to ensure the pressure instrumentation sense line for a steam generator pressure protection channel transmitter was not obstructed. Specifically, identified blockages in the sense line were not cleared and resulted in an inoperable ESFAS channel.

PLANT STATUS

Unit 1 began the inspection period at rated thermal power (RTP). On April, 14, 2019, the reactor tripped on number 3 steam generator low level following a loss of the 'A' main feedwater pump. The unit was returned to RTP on April 17, 2019 and remained at or near RTP for the remainder of the inspection period.

Unit 2 began the inspection period at rated thermal power and remained at or near RTP for the entire inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed plant status activities described in IMC 2515 Appendix D, Plant Status and conducted routine reviews using IP 71152, Problem Identification and Resolution. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

Green NCV 05000327/2019002-01 Open/Closed

[H.5] - Work Management 71111.15 The inspectors identified a green, non-cited violation of 10 CFR 50 Appendix B criterion XVI when the licensee failed to ensure the pressure instrumentation sense line for a steam generator pressure protection channel transmitter was not obstructed. Specifically, identified blockages in the sense line were not cleared and resulted in an inoperable ESFAS channel.

Description:

On April 14, 2019, Unit 1 reactor tripped on low steam generator level following the loss of a main feedwater pump. During the post trip review of this transient, the licensee noted that, following rapid pressure changes, one of the loop 3 steam generator pressure indicators, 1-PT-1-23, was lagging the indication of other steam generator pressure transmitters.

Specifically, the licensee noted pressure indication on the steam generators (except for 1-PT-1-23) increased from approximately 880 psig to 920 psig (40 psig) in less than a second and then remained steady for over a second while 1-PT-1-23 slowly increased by 16 psig over the same time period. Similarly, when steam generator pressures later increased approximately 60 psig in less than a second, 1-PT-1-23 slowly increased by 7 psig in the same time period.

The required response times for the steam pressure channel is 0.028 seconds. 1-PT-1-23 is used to monitor loop 3 steam line pressure and is one of three transmitters that are used with their instrument loops to provide the two-out-of-three (per steam line) logic required by Technical Specifications for Safety Injection and Steam Line Isolation. Due to this observed delayed response, operators declared 1-PT-1-23 inoperable.

It should be noted that, until the time of the trip on April 15, 2019, the licensee considered 1-PT-1-23 operable based on acceptable channel checks and calibrations. Prompted by questions raised by the inspectors, the licensee reviewed past operating experience which identified similar behavior of 1-PT-1-23 in 2015; this behavior was not recognized at that time and no corrective actions were taken. Plugging of steam generator pressure transmitter sense lines had been previously identified at Sequoyah. Condition Report (CR) 915880 was written in July 2014 documenting that steam generator instrument sense lines had been found plugged or partially plugged and erratic measurements had been attributed to partial sense line plugging. Actions were generated in the licensee's corrective action program to clear 1-PT-1-23 sense line to remove corrosion products during the upcoming refueling outage (1R20) and setup repetitive work orders for performance of this action. These actions were not performed.

Corrective Actions: 1-PT-1-23 was removed from service and the sense line was rodded out to remove accumulated material. 1-PT-1-23 was returned to operable status on April 16, 2019.

Performance Assessment:

Performance Deficiency: The licensees failure to ensure identified blockages in the instrumentation sense line for a steam generator pressure protection channel transmitter were removed was a performance deficiency.

Screening: The inspectors determined the performance deficiency was more than minor because it was associated with the Equipment Performance attribute of the Mitigating Systems cornerstone. Specifically, identified blockages in 1-PT-1-23 sense line were not cleared and resulted in an inoperable engineered safeguard feature actuation system (ESFAS) channel.

Significance: The inspectors assessed the significance of the finding using Appendix A, Significance Determination of Reactor Inspection Findings for At - Power Situations. The finding screened to green because all four questions in Exhibit 2, "Mitigating Systems Screening Questions" were answered "no".

Cross-Cutting Aspect: H.5 - Work Management: The organization implements a process of planning, controlling, and executing work activities such that nuclear safety is the overriding priority. The work process includes the identification and management of risk commensurate to the work and the need for coordination with different groups or job activities. The licensee created corrective action work orders several years prior to the event to clear the steam generator pressure transmitter sense lines. However, the work management process did not prioritize the execution of this work and these work orders were never performed.

Enforcement:

Violation: Title 10 CFR Part 50, Appendix B, Criteria XVI, Corrective Actions, requires, in part, that measures be established to assure conditions adverse to quality are promptly identified and corrected. Contrary to the above, from November 23, 2015 until April 14, 2019, the measures established by the licensee failed to correct an identified condition adverse to quality. Specifically, the licensee identified a condition adverse to quality, obstruction of a steam generator pressure transmitter sense line, but did not take action to correct the condition.

Enforcement Action: This violation is being treated as an non-cited violation, consistent with Section 2.3.2 of the Enforcement Policy.
